I WAS ON I-75 WHEN I ACCELERATED HARD WHILE TRYING TO MERGE INTO A GAP IN HEAVY, MERGING TRAFFIC. WHEN I TOOK MY FOOT OFF THE ACCELERATOR MY VAN CONTINUED TO ACCELERATE AS THE ENGINE CONTINUED TO REV AT A HIGH RPM. I STABBED AT THE ACCELERATOR WITH MY FOOT TO SEE IF I COULD UNSTICK IT, BUT IT ONLY CAUSED THE ENGINE TO REV EVEN HIGHER - TO FULL-THROTTLE. I JABBED AT IT MULTIPLE TIMES TO NO AVAIL. I TRIED TO LIFT THE GAS PEDAL WITH MY FOOT BY JAMMING IT UNDERNEATH THE PEDAL AND PULLING IT UP, BUT IT FLOATED FREELY AND CONTINUED TO FALL BACK TO ITS FULL-THROTTLE POSITION. ALTHOUGH I WAS CONFIDENT THAT THE FLOOR MAT WAS NOT THE CAUSE OF THE PROBLEM I PULLED IT AWAY FROM THE PEDALS AND, AS EXPECTED, THERE WAS NO EFFECT. I WAS ABLE TO KEEP CONTROL OF MY VAN BY APPLYING EXTREMELY HEAVY PRESSURE TO THE BRAKE WHILE SHIFTING BETWEEN NEUTRAL AND DRIVE. DURING THAT TIME I WAS STILL JABBING AND LIFTING THE ACCELERATOR WITH MY RIGHT FOOT TRYING TO 'UNSTICK' THE GAS PEDAL. I WAS ENTERING AND CROSSING AND OVERPASS THAT WAS UNDERGOING CONSTRUCTION. CONCRETE BARRIERS LINED BOTH SIDES OF THE HIGHWAY AND BRIDGE WHERE THE EMERGENCY LANES WOULD HAVE OTHERWISE BEEN LOCATED. I HAD TO MAINTAIN SOME SPEED, FOR FEAR OF BEING REAR-ENDED, WHILE SEARCHING FOR THE FIRST OPPORTUNITY TO PULL OVER. MY VEHICLE WAS TOWED TO THE NEAREST MAZDA DEALERSHIP WHERE THE MECHANIC WAS ABLE TO QUICKLY ASCERTAIN THAT THE SCREWING MECHANISM FROM AN UNSECURED AIR HOSE CLAMP HAD SNAGGED THE THROTTLE ARM - THE CLAMP ATTACHES THE HOSE TO THE ENGINE. AS I PUSHED HARDER ON THE GAS PEDAL IT FURTHER EXACERBATED THE PROBLEM BY LOCKING THE ARM IN A MORE WIDE-OPEN THROTTLE POSITION. THE CLAMP HAD BEEN LEFT UNSECURED FROM PREVIOUS WORK DONE AT A MAZDA DEALERSHIP. ACCORDING TO THE MECHANIC, THIS COULD HAVE ALSO OCCURRED IF THE CLAMP HAD BEEN SECURED, BUT NOT POSITIONED OUT OF THE WAY OF THE ARM. *TR

THE ENGINE COILS HAVE FAILED 4 TIMES SINCE WE HAVE OWNED THE VEHICLE. LAST NIGHT WHILE TRAVELING ON THE INTERSTATE IN THE RAIN, KEY WORDS, IN THE RAIN IT STARTED AGAIN. I AM SURE AT LEAST ONE IF NOT TWO COILS WENT THIS TIME. JUST HAD A COIL AND ALL THE BOOTS THAT ARE SUPPOSE TO PROTECT THE COILS AND KEEP THE MOISTURE OUT REPLACED IN FEBRUARY, 2012. IT ALWAYS HAPPENS WHEN IT RAINS. MAZDA IS WELL AWARE OF THE SITUATION AND HAS DONE NOTHING TO FIX THIS ISSUE EXCEPT TO STOP USING THIS SYSTEM AND START MAKING THEIR CARS DIFFERENTLY. BUT WHAT ABOUT THE PEOPLE WHO OWN THE OLDER MODELS? THIS IS A DANGEROUS SITUATION AS I WAS ON THE HIGHWAY, AT NIGHT, AND IT WAS POURING DOWN RAIN AND THE VAN STARTS SLOWING DOWN AND SPUTTERING ALONG, WITH TRAFFIC TRAVELING BEHIND ME. IGNITION COILS RETAIN MOISTURE AND CAUSE CYLINDERS TO MISFIRE. THIS CAUSES VEHICLES TO STALL OR LOSE POWER -- USUALLY WHEN RAINING. IN OUR CASE, THIS HAS HAPPENED REPEATEDLY ON THE INTERSTATE DURING RAIN. WE EARLY CAUSED AN ACCIDENTS BY OUR INABILITY TO GET OUT OF TRAFFIC WITH OUR SUDDENLY-SLOWED VEHICLE. REPLACEMENT OF A SINGLE COILS COST US OVER $200. THERE ARE SIX IGNITION COILS (OVER $1000). MANY INTERNET FORUMS INDICATE THAT THIS PROBLEM IS SYSTEMIC IN THIS MAKE/MODEL. WITH OUR MAZDA 2004 MPV, WE HAVE HAD FOUR SEPARATE INCIDENTS IN 2 1/4 YEARS FROM MAR 2008 THROUGH JUNE 2010 WHERE THE CHECK ENGINE LIGHT HAS COME ON TO INDICATE AN ENGINE PROBLEM. IN EACH INCIDENT, THE VEHICLE RUNS VERY ROUGH AND VERY SLOW. EACH INCIDENT RESULTED IN US TAKING THE VEHICLE TO THE MAZDA DEALERSHIP WHERE SOME OR ALL OF THE IGNITION COILS WERE REPLACED. FOR SOME OF THE INCIDENTS, THE VEHICLE WAS OUT OF WARRANTY BUT IN EACH INCIDENT ALL OR SOME OF THE COSTS WERE COVERED BY MAZDA. IN 2 OF THE INCIDENTS, WE WERE OUT OF TOWN AND IT WAS DARK. LIKE MANY OTHER MAZDA MPV OWNERS, I'VE EXPERIENCED PROBLEMS WITH THE VEHICLE'S IGNITION COILS. IN AUGUST 2008, THE VEHICLE WAS RIDING VERY ROUGH AND HESITATING ON THE HIGHWAY. THE DIAGNOSTICS FOR THE CHECK ENGINE LIGHT SHOWED THAT ONE OF THE IGNITION COILS HAD MISFIRED. I HAD THE COIL REPLACED AND, AT THE RECOMMENDATION OF THE DEALERSHIP, REPLACED THE SPARK PLUGS. AT THAT TIME, MY VEHICLE WITH APPROXIMATELY 35,000 MILES ON IT, WAS OUT OF WARRANTY. I PAID $476 FOR THE REPAIR BUT WAS ULTIMATELY REIMBURSED BY MAZDA ON A CUSTOMER GOODWILL BASIS. ONE YEAR LATER, IN AUGUST 2009, THE VEHICLE AGAIN BEGAN RIDING ROUGH AND HESITATING. WHEN THE CHECK ENGINE LIGHT CAME ON, I BROUGHT THE VEHICLE TO THE DEALERSHIP. BASED ON THE EARLIER EXPERIENCE, I SUSPECTED AN IGNITION COIL AS THE CULPRIT. MY SUSPICIONS WERE REALIZED. THE DEALERSHIP FOUND THAT CYLINDER #5 HAD MISFIRED AND THAT SEVERAL OTHER COILS WERE WEAK. THE DEALER INDICATED THAT THE OTHER COILS WERE LIKELY TO GO AND RECOMMENDED THAT ALL OF THEM BE REPLACED. I HAD THE WORK DONE AT A COST OF $960. THE VEHICLE AT THIS POINT HAD ONLY 46,000 MILES ON IT. BOTH REPAIRING DEALERSHIPS MENTIONED THAT THIS IS A COMMON PROBLEM WITH MPVS. FROM MY RESEARCH, I CAN SEE THAT IS A SIGNIFICANT PROBLEM FOR 2002-2006 MPV'S. ENTIRE MESSAGE BOARDS ARE DEDICATED TO THIS MAZDA MPV IGNITION COIL DEFECT. MOST BELIEVE THAT THERE'S A DESIGN FLAW THAT PREVENTS THE COILS FROM DISPERSING MOISTURE PROPERLY, WHICH OVER TIME LEADS THE CYLINDERS TO MISFIRE. IT SEEMS TO ME THAT MAZDA SHOULD HAVE VOLUNTARILY RECALLED THE VEHICLE DUE TO THIS ISSUE. HAVING PREVIOUSLY MADE A CUSTOMER ACCOMMODATION ON THE EARLIER REPAIR, MAZDA HAS REFUSED TO REIMBURSE ME FOR THE COSTS OF THIS LATEST REPAIR. 2 OCCURANCES IN 3000 MILES OF ENGINE LOOSING POWER, THEN STARTS TO BUCK ABOVE 3500-4000 RPM. THIS FIRST HAPPENED ON THE FREEWAY AND IT WAS DIFFICULT STAYING OUT OF THE WAY OF TRAFFIC UNTIL I COULD EXIT. THE SECOND OCCURANCE WAS AFTER I HAD EXITED THE FREEWAY ONTO A 2 LANE. I THEN DROVE 30 MILES TO HOME. BOTH TIMES IT WAS OK THE NEXT MORNING. DEALER CANNOT REPLICATE THE PROBLEM. I JUST HAD MY TIRES CHECKED AT 10,000 MILES DURING AN OIL CHANGE AT MY DEALERSHIP. NO PROBLEM WAS FOUND. I NOW HAVE 11,000 MILES ON MY CAR AND TIRES. MY TIRES ARE MICHELIN RADIAL XSE P215/50 R17. I WAS ON MY WAY TO WORK AND WITHOUT WARNING MY DRIVERS SIDE BACK TIRES BLEW OUT. IT SEPARATED FROM THE TIRE AND THE CORDS WERE SHOWING. I AM CONVINCED THAT THIS IS THE FAULT OF THE MICHELIN TIRE BECAUSE OF THE TREAD WEAR AND THE MILES ON IT. I DID NOT HIT ANYTHING IN THE ROAD NOR WAS THE TIRE PUNCTURED. WHEN THE TIRE BLEW I WAS ON WEST BOUND ON I-70 TO DOWNTOWN INDIANAPOLIS AND WITHOUT WARNING I SPUN AROUND SEVERAL TIMES IN THE MIDDLE OF I-70 AND GOT INTO AN ACCIDENT AND HAD TO BE TAKEN TO THE HOSPITAL VIA AMBULANCE FOR NECK AND BACK INJURIES. MY MAZDA 6 RECIEVED 6,000 DOLLARS IN DAMAGE THAT I KNOW OF SO FAR NOT TO MENTION THE PERSON THAT I CRASHED INTO. MY INSURANCE WILL NOT PAY FOR THE TIRE BECAUSE THAT WAS THE REASON FOR THE ACCIDENT. I THINK MICHELIN SHOULD PAY FOR 4 NEW TIRES OF A DIFFERENT BRAND, MY HOSPITAL VISIT, MY DEDUCTABLE AND MY RENTAL CARE EXPENSE AND MY LOST WAGES. ALSO MY TIRE PRESURE WAS CHECKED BY MY DEALER AGAIN THE WEEK OF JUNE 27,2005. I'M VERY LUCKY THAT I DIDN'T GET HURT WORSE OR SOMEONE GETTING HURT. ON DECEMBER 20, 2004, I TRIED TO CRANK MY NEW 2004 RX-8 TO GO TO WORK. IT WAS 19 DEGREES OUTSIDE. THE CAR WOULD NOT CRANK. IT HAD BEEN SITTING FOR 2 DAYS OVER THE WEEKEND. Mazda North America Operations (Mazda) is recalling certain 2003-2008 Mazda6, 2006-2007 Mazdaspeed6 and 2004 MPV vehicles nationwide. Mazda is also recalling 2005-2006 MPV vehicles in Alabama, Arizona, Arkansas, California, Delaware, the District of Columbia, Florida, Georgia, Hawaii, Illinois, Indiana, Kansas, Kentucky, Louisiana, Maryland, Mississippi, Missouri, Nebraska, Nevada, New Jersey, New Mexico, North Carolina, Ohio, Oklahoma, Pennsylvania, South Carolina, Tennessee, Texas, Virginia, West Virginia, Puerto Rico, American Samoa, Guam, the Northern Mariana Islands (Saipan), and the U.S. Virgin Islands. On July 9, 2019 this recall was expanded to include certain 2004-2011 RX-8 vehicles that were previously recalled under 16V-354 but have not yet been repaired. These vehicles are equipped with certain air bag inflators assembled as part of the passenger frontal air bag modules, and used as original equipment or replacement equipment. In the event of a crash necessitating deployment of the passenger frontal air bag, these passenger air bag inflators may explode due to propellant degradation occurring after long-term exposure to absolute humidity and temperature cycling.

Mazda North American Operations (Mazda) is recalling certain 2003-2008 Mazda6, 2004-2005 MPV, 2004 RX-8, and 2006-2007 Mazdaspeed6 vehicles. These vehicles had their passenger frontal air bag inflators previously replaced under a prior recall using inflators of the same design. The inflators may explode due to propellant degradation occurring after long-term exposure to high absolute humidity, temperature and temperature cycling.
