My rav4 caught fire by its self not running in the battery compartment and burned completely to the metal
The contact owns a 2012 Toyota Rav4. The contact stated while driving at an undisclosed speed, there was a delay for the vehicle to stop when the brake pedal was depressed. The contact had to put a lot of pressure while depressing the brake pedal for the vehicle to stop. An independent mechanic was made aware of the failure. The mechanic had repaired the brake master cylinder, front and rear brake-disc rotors, and caliper in 2019. The dealer was made aware of the failure and scheduled an upcoming appointment for diagnostic testing. The manufacturer was made aware of the failure and a case was opened. The failure mileage was 139,435.
Unknown. Vehicle is having problems with fuel system. Vehicle intermittently runs rough like engine is not getting enough gas.
LACK OF TIE POINTS: This is an issue with our RAV4, but it is an issue with about every vehicle manufactured recently. It is a requirement of drivers to “secure their load” but when top carrying boats it is very difficult to find suitable tie points in the front and rear of the vehicle. Could vehicle manufacturers be required to provide tie points suitable for securing loads? It could be as simple as providing smooth- edged holes in the chassis under both bumpers.

I HAVE JUST BEGUN HAVING THE ISSUE THAT MANY OTHERS COMPLAIN ABOUT - THE CHECK ENGINE LIGHT + 4WD + TRAC LIGHTS ALL CAME ON (2012 TOYOTA RAV4, 100,000 MILES). WE TRIED SOME SIMPLE FIXES AT HOME, BASED ON THE MANY ONLINE POSTS ABOUT THIS ISSUE (CHECKED/REFASTENED HOSES, BATTERY CONNECTIONS, ETC.), AND THE LIGHTS CAME RIGHT BACK ON. TOOK TO OUR USUAL MECHANIC, AND THEY DIAGNOSED O2 SENSOR PROBLEMS, BOTH BANK 1 & BANK 2. PAID $750 FOR THEM TO REPLACE BOTH SENSORS. THE 3 DASHBOARD LIGHTS CAME BACK ON AS I WAS DRIVING HOME AFTER PICKING UP MY CAR FROM THE SERVICE. THE SENSORS WERE REPLACED WITH GENUINE TOYOTA PARTS. THE CAR IS NOW BACK AT THE MECHANIC - THEY SAID THE SAME O2 SENSOR CODES ARE COMING UP WHEN THEY CHECK THE DIAGNOSTICS. THIS APPEARS TO BE A CHRONIC PROBLEM FOR TOYOTA, AND REALLY NEEDS TO BE INVESTIGATED AND DEALT WITH. I HAD THE SAME PROBLEM WITH AN EARLIER TOYOTA SIENNA, AND SPENT MUCH TIME AND MONEY TRYING TO FIX THE ISSUE, WHICH WAS NEVER RESOLVED. THERE ARE MORE THAN 15 YEARS OF COMPLAINTS ABOUT THIS ISSUE - PLEASE HELP US, THE CONSUMERS!
BOTH FRONT DRIVE AXLES EXHIBITED SIGNS OF SEVERE CORROSION DAMAGE LEADING TO COMPLETE LOSS OF LUBRICANT AND HAD TO BE REPLACED AT 34,056 MILES. THE VEHICLE IS GARAGED AND NOT SUBJECT TO ANY UNUSUAL OPERATING CONDITIONS. SYMPTOMS INCLUDED SEVERE CRACKING AND POPPING NOISES ON TURNS.
DRIVER'S SIDE SUN VISOR HAD TO BE REPLACED A COUPLE YEARS AGO BECAUSE IT WOULD NOT STAY IN AN UPRIGHT POSITION AND HUNG IN A LOOSE DROPPED POSITION.THE PASSENGER SIDE SUN VISOR IS NOW DOING THE SAME THING. THIS IS A SAFETY HAZARD AS IT CAN DROP UNEXPECTEDLY AND BLOCK THE DRIVERS VISUAL FIELD.

THE CONTACT WAS INFORMED THAT THE WINDSHIELD WIPER MOTOR PUMP WAS DEFECTIVE AND NEEDED TO BE REPAIRED. THE VEHICLE WAS NOT REPAIRED. THE LOW WINDSHIELD WASHER FLUID WARNING LIGHT WAS ILLUMINATED. THE MANUFACTURER WAS NOT CONTACTED. THE FAILURE MILEAGE WAS 150,000.

MY 4WHEEL DRIVE LIGHT TRACTION LIGHT AND ENGINE LIGHT CONE ON. I WAS AFRAID TO DRIVE IT BUT I HAD IT CHECKED OUT AND IT'S A SENSOR PROBLEM . THIS HAS BEEN A PROBLEM SINCE 2007
ON JUNE10TH. I HAD A MAJOIR ACCIDENT IN WHICH BOTH MY WIFE & MYSELF SUFFERED FRACTURED STERNUMS & FRACTURED RIBS. WHAT HAPPENED WAS THAT I PULLED INTO A LOT IN PRINCETON, N.J. & AFTER STOPPING TO SET MY GPS, I JUST TAPPED THE ACCELERATOR & THE CAR SHOT FORWARD, AS IF SHOT OUT OF A CANNON. WHILE STOPPED I PUT THE CAR IN REVERSE & THE CAR SHOT BACKWARDS IN THE SAME MANNER, THEREBY STRIKING A WALL. WHEN PULLING MY 2012 TOYOTA RAV4 INTO A PARKING SPACE AT VERY LOW RATE OF SPEED, I TAPPED THE BRAKE PEDAL AND THE ENGINE IMMEDIATELY STARTED ROARING AND ACCELERATING OUT OF CONTROL. THE CAR JUMPED THE CURB THAT WAS DIRECTLY IN FRONT OF THE PARKING SPOT, CROSSED THE ROAD, JUMPED ANOTHER CURB, AND LANDED IN A MUDDY RAVINE. THE CAR WAS TOWED TO THE TOYOTA DEALERSHIP TO BE REPAIRED AND THE TECH COULDN'T FIND ANYTHING WRONG WITH THE BRAKES. I REPORTED THE INCIDENT TO TOYOTA. THEY SENT AN INVESTIGATOR. THEY CLAIM THE EDR DETECTED NO DEFECTS IN THE VEHICLE AND BLAMED IT ON EITHER ME MISTAKING THE ACCELERATOR FOR THE BRAKES OR MY CUSTOM FITTED, LOCK-IN WEATHERTECH FLOOR MAT WHICH HAS BEEN IN PLACE SINCE DAY ONE OF OWNING THIS VEHICLE SINCE 2012. I HAD NEVER HAD ANY PROBLEMS WITH THE VEHICLE BEFORE THIS AND I HAVE ALWAYS LOVED MY CAR BUT AM NOW TERRIFIED THAT IT WILL ACCELERATE ON IT'S OWN AGAIN. *TR
IN A PAVED PARKING LOT I ATTEMPTED TO PARK BY APPLYING THE BRAKES BUT THE CAR SURGED FORWARD. IT ONLY STOPPED BY GOING OVER A CURB AND HITTING A TREE AT THE PERIMETER OF THE PARKING SPACE.
AIR BAG LIGHT IS ON ALL THE TIME AND SEAT BELL IS FLASHING AND FRONT ROTORS EXCESSIVE RUST AND ONLY ABOUT 2 1/2 INCHES IN MIDDLE THE ROTORS.
I NOTICED THAT THE 'PASS AIR BAG OFF' LIGHT WAS ON EVEN THOUGH A PASSENGER WAS SITTING IN THE FRONT PASSENGER SEAT. I TOOK IT TO THE TOYOTA DEALERSHIP THAT DIAGNOSED IT. THEY SAID THE SENSOR IN THE PASSENGER SEAT BELT BUCKLE HAD GONE BAD AND NEEDED REPLACING. WHAT BOTHERS ME IS THE FACT THAT THE AIRBAG LIGHT ON THE INSTRUMENT PANEL DIDN'T COME ON TO INDICATE THAT SOMETHING WAS WRONG. I AM NOT SURE HOW LONG I HAVE DRIVEN WITH THIS MALFUNCTION SINCE THE LIGHT THAT INDICATES THE AIR BAG BEING ON OR OFF IS OFF TO THE SIDE AND IN YELLOW RATHER THAN RED TO SHOW THAT SOMETHING IS WRONG. I HAD TO PAY FOR THE DIAGNOSIS AND REPAIR. THIS SHOULD HAVE BEEN COVERED BY AN EXTENDED WARRANTY BY THE MANUFACTURER. THE CAR ONLY HAD 67K MILES, BEING 7 YEARS OLD.

THE VEHICLE WAS NOT REPAIRED. THE MANUFACTURER WAS INFORMED OF THE FAILURE. THE CONTACT STATED THAT TOYOTA CORPORATION SENT AN ENGINEER TO RETRIEVE DATA FROM THE VEHICLE DATA RECORDER, BUT WAS UNSUCCESSFUL. THE APPROXIMATE FAILURE MILEAGE WAS 33,450. *LN *TP THE CONSUMER STATED INSPECTION WAS PERFORMED IN SEPT 2019 AND NO DEFECT WAS FOUND IN VEHICLE. AND LAST INCIDENT WAS ON DECEMBER 16TH AND BRAKES/TIRES WERE REPLACE A MONTH PRIOR. *PM
3 WEEKS AFTER THE 50,000 MILE SERVICE, ON OCTOBER 22 2019, I WAS DRIVING 35 MPH ON SOQUEL AVENUE, WHEN THE CAR BECAME UNRESPONSIVE, I LOST ALL POWER AND CONTROL OF THE VEHICLE, AND "CHECK EV SYSTEM" CAME ONTO THE INSTRUMENT CLUSTER. I WAS ABLE TO COAST TO THE SIDE OF THE ROAD INTO A BUS STOP, TURNED THE CAR OFF, AND HAD THE CAR TOWED INTO THE DEALERSHIP. THE DEALERSHIP WAS UNABLE TO REPLICATE THE SYMPTOM AND INFORMED ME THAT IT WAS DUE TO THE 12V BATTERY. THEY REPLACED IT AND ASSURED ME THE ISSUE WOULD NOT HAPPEN AGAIN. ON NOV 11, 2019, THE ISSUE HAPPENED AGAIN WHILE I WAS ON THE HIGHWAY THIS TIME. I HAD THE CAR TOWED IN AGAIN TO TOYOTA, WHO INFORMED ME THAT "SOME RAV4 EVS HAVE BEEN IN THE SHOP FOR MONTHS, UNABLE TO BE DIAGNOSED". I RESEARCHED THE SYMPTOM ONLINE AND FOUND THAT IT RELATED TO A 2015 RECALL THAT RELATED TO THE ELECTRIC TRACTION MOTOR. TOYOTA'S DESCRIPTION OF THE F01 RECALL: IN THE INVOLVED RAV4 EV'S, COMPONENTS IN THE ELECTRIC VEHICLE TRACTION MOTOR ASSEMBLY, WHICH IS PART OF THE PROPULSION SYSTEM, MAY CAUSE THE VEHICLE TO SHIFT TO 'NEUTRAL' DUE TO A SOFTWARE ISSUE. THIS CONDITION WILL ALSO TRIGGER A 'CHECK EV SYSTEM' WARNING MESSAGE ON THE INSTRUMENT PANEL AND TURN ON A MALFUNCTION INDICATOR LAMP. IF THE VEHICLE SHIFTS TO 'NEUTRAL', THIS WILL RESULT IN A COMPLETE LOSS OF DRIVE POWER, WHICH CAN INCREASE THE RISK OF A CRASH. I INFORMED THE DEALERSHIP THAT MY SYMPTOMS MATCHED UP WITH RECALL EXACTLY, AND THEN WAS TOLD THAT IT HAD NOTHING TO DO WITH THIS AS THIS RECALL HAD BEEN PERFORMED ON APRIL 18 2016 WHEN I HAD MY CAR IN. UPON FURTHER QUESTIONING, I WAS ALSO TOLD THAT SOME OF THE RAV4 EVS HAD HAD THEIR MOTORS REPLACED AS PART OF THIS RECALL, IN ADDITION TO THE SOFTWARE CHANGE, BUT MINE HAD NOT. TOYOTA FAILED TO RECOGNIZE THIS FLAW NOT ONCE BUT TWICE AND I AM SEEKING COMPENSATION FOR FAILURE TO PROPERLY RECOGNIZE AND ADDRESS A KNOWN ISSUE.

MY RAV4 ACCELERATES EXCESSIVELY OUT OF THE BLUE, IN A DANGEROUS MANNER FAR EXCEEDING ANY NEED TO MAINTAIN THE SET SPEED. SLAMMING ON THE BRAKES IS THE ONLY WAY TO STOP IT IS HAPPENING MORE FREQUENTLY. IN A DAY AND A HALF OF HIGHWAY DRIVING IT HAPPENED THREE SEPARATE AND RANDOM OCCURRENCES YESTERDAY, SEP 4TH AND TODAY SEP 5TH, 2019. FOR EXAMPLE I WAS DRIVING ON I-44 EASTBOUND AT CRUISE CONTROL OF 70 MPH. AND AS I WAS GOING ALONG IT HAD ACCELERATED TO 75 BEFORE I RECOGNIZED WHAT WAS HAPPENED AND SLAMMED AGGRESSIVELY ON THE BRAKE PEDAL OR IT WOULD HAVE SURPASSED 85 MPH EASILY. IT PUT ME IN DANGER OF HAVING AN SERIOUS ACCIDENT WITH A SEMITRAILER IN JULY. I SOUGHT ANSWERS FROM A MECHANIC TODAY. HE SAID IT SOUNDED SIMILAR TO WHAT HAS HAPPENED TO OTHER TOYOTA MODELS. I TURNED THE WIPERS ON THE OTHER DAY WHEN IT WAS RAINING, THEY CAME ON AND MADE ONE WIPE UP AND STOPPED ON THE DOWNWARD WIPE. I GOT OUT AND PULLED ON ONE SIDE AND THEY WIPED SEVERAL MORE TIMES THEN STOPPED AGAIN. I PULLED ON THEM AGAIN AND THEY MOVED BUT VERY SLOWLY THEN THEY STOPPED COMPLETELY. PURCHASE NEW OFF LOT. VEHICLE BARELY USED. 2012 ONLY HAS SLIGHTLY OVER 26,000 ORIGINAL MILES. KEPT COVERED WHEN NOT IN USE. PAINT IS JUST POPPING ON IN SEVERAL LOCATIONS RIGHT DOWN TO THE PRIMER LAYER. STARTED NEAR BACK WINDOW, THEN WHEEL WELL AREA, ROOF SPOTS. WE HAVE PUT TOUCH UP PAINT ON THE BACK WINDOW THINKING WE COULD SLOW THE PROCESS BUT IT KEEPS GETTING BIGGER. 2012 RAV4 - FOR NO APPARENT REASON MY CHECK ENGINE LIGHT, TRAC OFF LIGHT AND 4X4 LIGHT HAVE BEEN COMING ON FOR A FEW DAYS, THEN OFF, THEN ON, ETC. WE GOT A CODE OF P0607, BUT THE CAR IS SHOWING NO SYMPTOMS ASSOCIATED WITH THAT CODE ERROR. THE VEHICLE IS DRIVING FINE. I HAVE CHECKED THE BATTERY CONNECTION AND THE GAS CAP. BASED ON A LARGE NUMBER OF ONLINE COMPLAINTS, I WOULD ASK THAT NHTSA SERIOUSLY LOOK INTO THIS ISSUE AND STRONGLY CONSIDER A RECALL THAT WILL ACTUALLY FIX THE PROBLEM FOR OWNERS. I WOULD MOVE IT TO THE SIDE BECAUSE I COULDN'T SEE TO DRIVE. THIS LAST TIME I TRIED TO MOVE IT TO THE SIDE IT COMPLETELY FELL OFF. MY PASSENGER SIDE IS DOING IT AS WELL AND MAKING IT HARD FOR THE PASSENGERS TO SEE AND EVEN MYSELF BECAUSE IT'S CONSTANTLY BLOCKING MY VIEW OF THAT SIDE. I'M VERY AGGRAVATED BECAUSE I CALLED CUSTOMER CARE AND THEY WILL NOT DO ANYTHING ABOUT IT. THIS IS A VERY DANGEROUS SAFETY HAZARD. I'M VERY DISAPPOINTED IN TOYOTA! THE MANUFACTURER WAS NOT NOTIFIED OF THE FAILURE. THE FAILURE MILEAGE WAS 98,000.
AS I WAS DRIVING ON A HIGHWAY GOING STRAIGHT WITH A SPEED OF APPROX. 60 MPH, I HEARD A STRANGE ENGINE NOISE. I SAW THE OIL PRESSURE LIGHT FLICKERING AND THEN I EXITED THE HIGHWAY, PARKING ON A NEARBY STREET. I CHECKED THE OIL AND THE DIP STICK WAS DRY. MY CAR WAS TOWED TO A AUTO SHOP. THE FIRST SHOP ( NOT TOYOTA) DETERMINED THE OIL LEAK WAS FROM THE OIL FILTER CAP O-RING WHICH PART OF IT FLATTEN AND EXTENDED OUT CAUSING A LARGE AMOUNT OF OIL TO LEAK OUT. THE SECOND SHOP( NOT TOYOTA) WHO DID THE LAST OIL CHANGE ( UNKNOWN BRAND OF FILTER KIT USED) REPLACED THE ORIGINAL PLASTIC OIL FILTER CAP WITH AN AFTER MARKET METAL ONE AND REFILLED APPROX. 3 QUARTS OF OIL. I WAS NEVER TOLD WHAT CAUSED THIS PROBLEM WITH THE OIL FILTER CAP O-RING. SINCE THEN THE CAR HAS BEEN RUNNING FINE BUT I'M CONCERNED THIS MIGHT HAPPEN AGAIN.
I TOOK MY VEHICLE IN FOR SERVICE DUE TO AN ENGINE KNOCK. AFTER 90 MINUTES TO TROUBLESHOOT I WAS TOLD THAT THERE WAS A MAJOR PROBLEM AND THAT MY VEHICLE WAS NOT SAFE TO DRIVE. ENGINE MOUNT WAS BROKEN, NEEDED REAR DIFFERENTIAL REPLACED. MECHANIC HAD NEVER SEEN ANYTHING LIKE THIS.

I WAS COMING TO A STOPLIGHT AND PRESS THE BRAKES THE CAR CAME TO STOP THEN THE ENGINE REVVED UP VERY FAST. MY FOOT WAS ON THE BRAKES BUT THE ENGINE WAS REVVING VERY FAST THAT I HAD TO USE THE PARKING BRAKE TO STOP THE VEHICLE FROM MOVING FORWARD. I PUT THE CAR IN PARK THEN THE ENGINE STOP REVVING.
I WAS COMING TO A STOPLIGHT AND I PUT MY FOOT IN THE BRAKE PEDAL. THE CAR SLOWED DOWN, CAME TO A STOP BUT THEN WITH MY FOOT ON THE BRAKE PEDAL, THE ENGINE REVED UP TO 3000 RPM. THE BRAKES WERE BARELY ABLE TO KEEP THE CAR STOPPED SO I HAD TO PUT THE EMERGENCY BRAKE ON. I WAS IN DRIVE THE WHOLE TIME. ONCE THE LIGHT CHANGED, I TOOK MY FOOT OFF THE BRAKE PEDAL AND ONTO TO GAS PEDAL AND THE RPM'S DROPPED BACK TO 1000. IT HASN'T HAPPENED SINCE THEN. TOOK IT TO MY MECHANIC AND SAID THEY COULDN'T REPLICATE THE PROBLEM SO THEY COULDN'T FIX ANYTHING.

THE AIRBAG LIGHT REMAINS ON. I HAVE BEEN INFORMED THAT THIS MEANS THAT IN CASE OF A VEHICLE CRASH MY AIRBAGS WILL NOT DEPLOY. MY VIN IS NOT PART OF THE RECALL, YET I AM EXPERIENCING THE SAME PROBLEM AS OTHER RAV 4 OWNERS THAT HAVE VIN NUMBERS WITHIN THE RECALL. THE TOYOTA DEALERSHIP WANTS TO CHARGE ME $88.00 PER HALF HOUR TO DIAGNOSE THE PROBLEM. THAT'S NOT FAIR TO ME.
VEHICLE EXPERIENCED SUDDEN UNINTENDED ACCELERATION WHEN EXITING THE HIGHWAY AND MAKING A TURN. RPM WENT UP TO 7,000 UNTIL THE VEHICLE WAS TURNED OFF. THE ACCELERATOR PEDAL WAS DEPRESSED FULLY SEVERAL TIMES WHILE THE CAR WAS SUSPENDED AND TURNED OFF AND STARTED BACK ON WITH THE BRAKE PEDAL FULLY DEPRESSED TO ENSURE PROPER SAFETY MEASURES IN CASE THE VEHICLE JOLTS FORWARD. NO ACCIDENT OR INJURIES OCCURRED DURING THE INCIDENT AS THE VEHICLE WAS PUT TO A STOP SAFELY BEFORE OTHER PARTIES WOULD BE INVOLVED. I WAS IN A PARKING AREA LOOKING FOR A PARKING SPOT. I HAD JUST DRIVEN UP A SLIGHT INCLINE AND CRESTED THE TOP WHEN I SPOTTED AN OPEN SPACE ON THE LEFT. I BRAKED SLIGHTLY TO TURN INTO THE SPOT, LIFTED MY FOOT OFF THE BRAKE BUT KEPT IT COVERED AND MADE THE LEFT TURN INTENDING TO COAST INTO THE SPOT. IN THE MIDDLE OF THE TURN THE VEHICLE INEXPLICABLY ACCELERATED AT WHICH POINT I SLAMMED MY FOOT ON THE BRAKE ENDING UP CROSS WAYS IN THE PARKING SPOT AND JUST SHORT OF A PARKING SIGN IN FRONT ON THE CURB. THE ENGINE IMMEDIATELY RETURNED TO IDLE SPEED. THE THROTTLE PEDAL, WHICH IS TOP MOUNTED, WAS EXAMINED AND FOUND TO BE CLEAR AND FREE. THE CONTACT COMMENTED THAT IT MAY HAVE BEEN MORE NOTICEABLE TO HER BECAUSE SHE WAS SHORT IN STATURE. HAVING READ ABOUT OTHER RAV4 OWNERS WITH THE SAME ISSUE, IT SEEMS LIKE A TOYOTA QUALITY CONTROL ISSUE.

WHEN MAKING A LEFT TURN FROM A STAND STILL THE BACK END OF THE VEHICLE FEELS VERY LOOSE AND WOBBLY AS IF A WHEEL WAS ABOUT TO COME OFF. IN ADDITION THE VEHICLE IS PRODUCING A VIBRATING OR THUMPING THAT APPEARS TO BE COMING FROM THE BACK END OF THE VEHICLE. THIS SEEMS TO BE A PROGRESSING ISSUE. MY 2012 RAV4 MAIN EVAP CANISTER WENT BAD AT ONLY 77,113 MILES. IT ALMOST COSTED ME $1550 TO GET THIS PART REPLACED, BUT THANKFULLY WAS ABLE TO CONVINCE THE DEALERSHIP TO GET ME A DEAL ON THE REPAIR. THERE IS NO REASON FOR THIS PART TO GO BAD SO QUICKLY. I DO NOT TOP OFF MY GAS AND I HAVE HAD IT IN FOR ROUTINE MAINTENANCE AT THE DEALERSHIP SINCE IT WAS PURCHASED. TO MAKE THE SITUATION WORSE, WHEN THE CHECK ENGINE LIGHT COMES ON, THE 4WD AND TRACTION CONTROL UNITS TURN OFF, MAKING MY CAR HARD TO CONTROL IN THE SNOW. I WAS TOLD BY TUFFY THIS IS A COMMON THING AND THAT WAS A PREVIOUS RECALL FOR OLDER VEHICLES. THE CODE WAS ON FOR A FEW WEEKS, DIDN'T CLEAR. I HAD IT DIAGNOSED AT TUFFY AND THE DEALERSHIP, SAME DIAGNOSIS. I HAVE ALSO HAD A PROBLEM WITH THE TRANSMISSION JERKING WHEN I PUT THE CAR INTO DRIVE, EVEN IN WARM WEATHER, WHEN IT'S THE FIRST DRIVE OF THE DAY. SUNROOF GLASS CRACKED WITH A LOUD BANG WHILE DRIVING ON HIGHWAY, AT ABOUT 55 MPH. WEATHER WAS CLEAR AND WARM (NOT VERY HOT). NO OTHER VEHICLES CLOSE BY. NO FLYING DEBRIS OBSERVED PRIOR TO LOUD BANG. ENTIRE PANE WAS RIDDLED WITH CRACKS. DURING SUBSEQUENT TRIP INTO TOWN, SOME OF THE GLASS PIECES CAME LOOSE AND FLEW OFF, LEAVING A LARGE HOLE SURROUNDED BY UNSTABLE GLASS.
PASSENGER SIDE MIRROR EXPLODED SPONTANEOUSLY LEAVING BEHIND ONLY ITS ASSEMBLY. VEHICLE WAS DRIVING AT 15 MPH THRU A SMALL TOWN WITH NO TURNS.
THE CHECK ENGINE, TRAC OFF AND 4WD LIGHTS WILL ALL COME ON TOGETHER AND STAY ON FOR A DAY OR TWO AND THEN GO OFF AGAIN. THIS KEEPS HAPPENING
WE RECEIVED A RECALL NOTICE AND BEEN CALLING THE DEALER REPEATEDLY. THEY STATE AS OF TODAY THEY STILL CAN NOT CORRECT THE DEFECT AS THEY HAVE NO PARTS AND HAVE NOT BEEN TRAINED. NOR WILL THEY SCHEDULE TO REPAIR INTO THE FUTURE. THIS RECALL EFFECTS THE SEAT BELTS IN THE BACK SEAT, WHICH IS WHERE MY CHILDREN SIT.

THE ATTEMPTED TO CALL THE MANUFACTURER, BUT THE WAIT TIME WAS LONG AND THE LINE DISCONNECTED. THE CONTACT HAD NOT EXPERIENCED A FAILURE. PARTS DISTRIBUTION DISCONNECT.
POWER DOOR LOCK ACTUATORS IN ALL DOORS HAVE FAILED. THE DRIVERS DOOR ACTUATOR FAILED FIRST. THE ACTUATORS IN EACH REMAINING DOOR INCLUDING THE TAILGATE DOOR HAVE SUBSEQUENTLY FAILED AS WELL. (THE DATE USED IN THIS REPORT TO INDICATE WHEN THE PROBLEM HAPPENED IS APPROXIMATE AND REFERS TO THE INITIAL PROBLEM WITH THE DRIVER'S DOOR.) TWO DIFFERENT TOYOTA SERVICE CENTERS HAVE DETERMINED THE ACTUATORS NEED TO BE REPLACED. ONE DEALER NOTED THAT THEY HAD SEEN THIS ISSUE IN MANY OTHER TOYOTA VEHICLES. A QUICK INTERNET SEARCH TURNS UP SEVERAL PAGES DISCUSSING THIS ISSUE. A SEARCH OF THIS SITE ALSO REVEALED SIMILAR COMPLAINTS WITH OTHER MODEL YEAR RAV4'S. EACH TOYOTA SERVICE CENTER ESTIMATED A COST OF APPROXIMATELY $400 FOR EACH ACTUATOR PLUS LABOR COSTS TO REPLACE THE PART. IT IS IMPORTANT TO NOTE THAT WHILE THE FRONT AND REAR DRIVERS-SIDE AND PASSENGER-SIDE DOORS HAVE MANUAL LATCHES INSIDE THE VEHICLE, THE TAILGATE DOOR DOES NOT. THE LACK OF A MANUAL LATCH AND KEYED LOCK IN THE TAILGATE DOOR MEANS THAT THE TAILGATE DOOR CANNOT BE OPENED AT ALL. THIS IS VERY PROBLEMATIC FOR BOTH CONVENIENCE AND SAFETY, PARTICULARLY IN THOSE RAV4'S WITH THE THIRD ROW SEAT. IN THE EVENT OF AN ACCIDENT OR VEHICLE FIRE, PASSENGERS IN THE THIRD-ROW SEAT WOULD BE FORCED TO EITHER EXIT THROUGH ONE OF THE OTHER DOORS OR BREAK A WINDOW TO EXIT THE VEHICLE. WHEN I CONTACTED TOYOTA REGARDING THIS ISSUE, I WAS TOLD INITIALLY TO HAVE MY LOCAL TOYOTA SERVICE CENTER EVALUATE THE PROBLEM AND CALL BACK TO TOYOTA TO DISCUSS AND SEE IF TOYOTA WOULD BE ABLE TO HELP AT ALL. WHEN I CALLED BACK TO TOYOTA, I WAS PUT ON HOLD FOR AN INCONVENIENT AMOUNT OF TIME, THEN SIMPLY TOLD THAT TOYOTA WOULD NOT HELP, PRESUMABLY BECAUSE MY VEHICLE'S WARRANTY HAS EXPIRED. I HOPE THAT TOYOTA WILL WORK WITH THE NHTSA TO TAKE A PROACTIVE APPROACH TO RECTIFY THIS PROBLEM FOR ALL AFFECTED TOYOTA VEHICLES AND OWNERS.
2013 TOYOTA RAV4 W/ 40K MILES HAD THIRD ACTUATOR POWER DOOR LATCH FAIL ON A THIRD DOOR IN 5 MONTHS. LOOKING AT OTHER RECENT COMPLAINTS, OF A SIMILAR NATURE, THIS MAY INDEED BE THE RESULT OF A MANUFACTURING DEFECT. PLEASE CONSIDER ISSUING A RECALL. MOST RECENT FAILURE IS THE DOOR MY 2 YEAR OLD USES, I WOULD HATE TO THINK OF AN EMERGENCY WHERE I COULDN'T GET HER OUT OF THE CAR BECAUSE IT WAS LOCKED. WHEN THE FIRST DOOR, DRIVER, WENT OUT (DEC 2016), I HAD IT FIXED AT MY MECHANIC THINKING IT WAS A FLUKE. WHEN THE SECOND DOOR, PASSENGER, WENT OUT MAR 2017, I CONTACTED TOYOTA & WAS TOLD TO GET THE PROBLEM CONFIRMED BY THE DEALERSHIP, WHICH I DID. THEY OFFERED ME A GOODWILL PYMT FOR THE SECOND DOOR ONLY, NOT THE FIRST, DESPITE HOW RECENTLY THE FIRST DOOR FAILED. IN MAY 2017 THE THIRD DOOR, BACK DRIVER, IS NOW FAILING & THE TOYOTA CUSTOMER SERVICE REP SAID THEY "ARE DECLINING TO OFFER ME ANY ASSISTANCE IN FIXING THIS ACTUATOR OR ANY FUTURE ACTUATOR LOCKS BECAUSE THEY GO OUT LIKE LIGHTBULBS EVERY 4 YEARS." I CANNOT BELIEVE TOYOTA KNOWINGLY MANUFACTURES VEHICLES W/ A PART THAT IT 'EXPECTS' TO FAIL EVERY 4 YEARS, LEAVING THEIR CUSTOMERS WITH A $2500 BILL THAT FREQUENTLY. WE ESCALATED THIS FROM THE INITIAL "CUST SERVICE" REP, ALEX S., TO HIS SUPERVISOR, ASHLEY, AND THEY BOTH SAID THEY ARE CHOOSING NOT TO HELP US ANY FURTHER WITH THIS PROBLEM ON DOOR NUMBER 3 (OR 1) OR DOORS 4 AND 5 WHEN THEY INEVITABLY GO OUT AS WELL. BUT, I ASKED TO INSPECT MY CAR IN A TOYOTA SERVICE CENTER, AND A MECHANIC SAID THAT THE REAR STRUTS AND SUSPENSIONS ARE WORN OUT, AND SHOULD BE REPLACED. IN ADDITION, HE SAID THAT THE FRONT SHOCK ABSORBERS WOULD SHOW A MALFUNCTIONING IN A NEAR FUTURE. THE MECHANIC SHOOK MY CAR WITH HIS HAND, AND MY CAR WAS SHAKEN ACCORDING TO HIS HAND. THE MECHANIC SAID THAT CARS SHOULD NOT BE SHAKEN BY HANDS. IT IS VERY DISAPPOINTED WITH TOYOTA AUTO. I SHOULD SPEND MORE THAN 2,000 DOLLARS TO FIX THE FRONT AND REAR SHOCK ABSORBERS WITH AFTER-MARKET PRICE. I HEARD THAT TOYOTA RECALLED FOR REAR SUSPENSION PROBLEM 2006~2011 RAV4. I TOOK IT IN TO THE DEALERSHIP AND THEY PUT NEW BRAKE PADS AND RESURFACED THE FRONT ROTORS. 42,968 MILES ON THE CAR.. I WAS TOLD THESE NEW BRAKE PADS HAD A LIFE TIME WARRANTY ON THEM.. AGAIN DEC. 2016 I STARTED HAVING THE SAME ISSUES SO I HAD TO TAKE THE RAV 4 BACK IN FOR SERVICE AND I WAS TOLD THAT THE BRAKE WARRANTY WOULD NOT COVER THE BRAKES UNTIL THE BRAKE PADS ARE WORN DOWN MORE. MY 2012 RAV4 EV HAS AN UNSAFE CONDITION WHERE THE ELECTRONIC TRANSMISSION DOES NOT ACTUALLY ENGAGE FROM R TO D, OR D TO R WHEN STATIONARY, EVEN THOUGH THE VEHICLE INSTRUMENTATION INDICATES SO. THE VEHICLE INSTEAD MOVES INADVERTENTLY IN THE WRONG DIRECTION. IT HAS BEEN A CHRONIC ISSUE WITH THE VEHICLE, AND I HAVE REPORTED IT TO THE TOYOTA DEALER ON SEVERAL OCCASIONS. EACH TIME I WAS TOLD THAT THE TECHNICIAN COULD NOT DUPLICATE THE CONDITION, BUT THAT A SOFTWARE UPDATE WOULD BE INSTALLED AND MIGHT CORRECT THE MATTER. I INITIALLY HAD TROUBLE GETTING THE CAR TO ENGAGE IN REVERSE OR DRIVE. I TURNED THE CAR OFF AND RESTARTED IT (REBOOTING THE SOFTWARE). THIS TIME REVERSE ENGAGED AND I MOVED THE CAR BACKWARD TO MAKE ROOM TO EXIT THE PARKING SPACE AND CAME TO A COMPLETE STOP. I THEN PUT THE CAR IN DRIVE, HEARD THE REVERSE 'BEEP' STOP, VISUALLY CHECKED THE INSTRUMENTATION CLUSTER TO VERIFY THE INDICATOR WAS ON 'D', APPLIED THE HILL-ASSIST FUNCTION, AND PUSHED THE ACCELERATOR TO DRIVE FORWARD. THE CAR INSTEAD LUNGED BACKWARD AND SLAMMED INTO THE CAR BEHIND ME. THIS IS A SERIOUS AND DANGEROUS CONDITION. IT COULD HAVE EASILY BEEN A PEDESTRIAN BEHIND ME INSTEAD OF A CAR. I GOT OUT OF MY CAR AND FORGOT TO PUT IT IN PARK. BECAUSE THE CAR HAS KEYLESS START, I WAS ABLE TO LEAVE THE CAR WITHOUT TAKING A KEY OUT. THE CAR STARTED TO ROLL BACK AND BECAUSE I LIVE AT THE TOP OF A HILL, IT BEGAN TO PICK UP SPEED. MY NEIGHBOR LIVES AT THE BOTTOM OF MY DRIVEWAY AND THE CAR WAS HEADING STRAIGHT TO THEIR HOUSE. LUKILY, THE CAR HIT A TREE WHICH STOPPED IT FROM HITTING THEIR HOUSE AND POTENTIALLY KILLING SOMEONE. THE ADJUSTER JUST TOLD ME MY CAR IS TOTALED DUE TO THE SPEED AT WHICH IT HIT THE TREE. HOWEVER, HE ALSO INDICATED THAT THE SPEED AT WHICH THE CAR WAS TRAVELLING WHEN IT HIT THE TREE, IT MAY HAVE HIT THE NEIGHBOR'S HOUSE AND CONTINUED THROUGH THE OTHER SIDE. THIS IS A SIGNIFICANT SAFETY FLAW OF WHICH NO ONE I SPEAK TO ABOUT IT IS AWARE. THEY ACTUALLY TEST IT WHICH THEIR OWN CAR AND NOTICE THE SAME ISSUE. IN MY OPINION, THERE SHOULD BE A MODIFICATION TO THIS FEATURE TO PREVENT THIS ISSUE. ESPECIALLY IS THERE ARE KIDS NEAR THE CAR.

ON OCTOBER 17 SHORTLY BEFORE 2 PM PST IN CLEAR WEATHER, I WAS PULLING INTO A PARKING SPOT ON PRIVATE PROPERTY AT ABOUT 2 TO 3 MPH. I TURNED THE VEHICLE INTO THE SPOT AND APPLIED THE BRAKES BUT THE CAR SURGED OVER THE CURB, HIT A TREE, WENT OVER AN EMBANKMENT AND CRASHED ONTO A PARKED CAR. (THE AIRBAG DID NOT DEPLOY, BUT THERE WERE NO INJURIES.) THE IGNITION WOULD NOT TURN OFF AFTER REPEATED ATTEMPTS. FINALLY I DEPRESSED THE IGNITION BUTTON FOR ABOUT 10 SECONDS AND IT TURNED OFF. JUST 5 DAYS EARLIER, THE CAR HAD UNDERGONE A SAFETY RECALL REPAIR TO THE ELECTRONIC CIRCUITRY. TOYOTA HAD ISSUED A SAFETY RECALL DUE TO A SOFTWARE DEFECT WHICH AFFECTED "COMPONENTS IN THE ELECTRIC VEHICLE TRACTION MOTOR ASSEMBLY." WE INFORMED TOYOTA THIS APPEARED TO BE AN UNINTENDED ACCELERATION INCIDENT. THEY PROMISED TO ASSIGN AN ENGINEER TO THOROUGHLY INSPECT THE VEHICLE AND DOWNLOAD INFORMATION FROM THE EVENT DATA RECORDER (EDR). THE ENGINEER INSPECTED THE CAR AND PREPARED A 22 PAGE REPORT. HOWEVER HE WAS UNABLE TO DOWNLOAD THE EDR INFORMATION DUE TO A DATA COMMUNICATION ERROR. TOYOTA CLAIMED IT WAS DUE TO A LOW BATTERY. THE ENGINEER CONFIRMED TO ME THAT IT WAS A DATA COMMUNICATION ERROR AND THAT THE VEHICLE WAS HALF CHARGED WHEN HE DROVE IT. TOYOTA REFUSED TO SHARE THE ENGINEER'S REPORT. THEY STATED IN SIX SHORT SENTENCES THAT THE ACCELERATION AND BRAKE PEDALS BOTH WORKED. THEY DID NOT ADDRESS THE RECALL, THE INCIDENT WITH THE IGNITION SWITCH, OR THE DATA COMMUNICATION ERROR, ALL OF WHICH SIGNAL POSSIBLE PROBLEMS WITH THE ELECTRIC CIRCUITRY/SOFTWARE. WHEN WE WROTE THEM A DETAILED LETTER WITH ALL THE ATTACHMENTS TO DOCUMENT THE RECALL, THEY SIMPLY RESPONDED THAT THE ACCELERATION PEDAL AND BRAKE PEDAL WORKED.
12/21/15 - MY VEHICLE WAS STATIONARY. I JUST FINISHED REFUELING AND STARTED THE CAR. I HAD MY RIGHT FOOT ON THE BRAKE, AND SWITCHED FROM PARK TO DRIVE. MY LEFT FOOT STAYED ON THE FLOOR, TO THE LEFT OF THE BRAKE PEDAL. WITHOUT REMOVING MY RIGHT FOOT FROM THE BRAKE, THE ENGINE REVVED AND THE CAR LURCHED FORWARD. I PRESSED EXTREMELY HARD ON THE BRAKE TO KEEP THE CAR FROM MOVING FORWARD, AND COULD HEAR AND FEEL THE ENGINE AND BRAKE FIGHTING EACH OTHER. THIS LASTED APPROXIMATELY 3 SECONDS AND THE ENGINE RETURNED TO IDLE. THE BRAKE AND ACCELERATOR PEDALS ARE NOT THAT CLOSE, IT WOULD BE IMPOSSIBLE TO PRESS BOTH AT THE SAME TIME.

I WAS TRAVELLING NORTHBOUND ON I-85 IN THE CITY OF ATLANTA AT AROUND 55 MILES PER HOUR. A CAR FROM THE HOV LANE ABRUPTLY CHANGED LANES AT HIGH SPEED AND HIT ME ON THE DRIVERS SIDE. THIS WAS A BIG IMPACT AND DUE TO THIS MY CAR SWERVED TO THE RIGHT LANE AND HIT ANOTHER MOVING TRUCK ON ITS DRIVERS SIDE. THE CAR HAS BEEN TOTALED BY MY INSURANCE COMPANY. MY CONCERN IS THAT, DUE TO THE NATURE OF THE ACCIDENT AND THE IMPACT THE AIR BAGS OF THE CAR SHOULD HAVE DEPLOYED AND THEY DIDN'T. THIS I THINK IS A LAPSE IN THE SAFETY OF THE CAR AND IS A MATTER OF LIFE AND DEATH. THE ACCIDENT TOOK PLACE ON 10/17/2015 AROUND 7:30 AM ON I-85N IN THE CITY OF ATLANTA. I HAVE ATTACHED THE POLICE REPORT FOR REFERENCE. PLEASE INVESTIGATE THE SAME.
THIS HAS HAPPENED ABOUT THREE TIMES. WHEN I AM STOPPING THE CAR WITH MY FOOT ON THE BRAKE THE ENGINE SUDDENLY REVS UP AND I HAVE TO REALLY EXERT PRESSURE ON THE BRAKE TO PREVENT THE CAR FROM LURCHING AHEAD. AFTER A FEW SECONDS OF BRAKING, THE ENGINE STOPS REVVING.
THE DRIVERS SIDE VISOR KEEPS FALLING DOWN AND BLOCKING MY VIEW WELL DRIVING. THE VISOR SEEMS TO BE DEFECTED OR POORLY MADE.
WHEN COMING TO A STOP AND MY FOOT IS ON THE BRAKES, THE CAR CONTINUES TO MOVE FORWARD. THIS HAS HAPPENED AT LEAST A DOZEN TIMES: 3 TIMES GOING DOWN A BRIDGE, TWICE ON THE INTERSTATE, AND THE REST IS WHEN I'M GOING DOWN A HILL AND PUT MY BRAKES ON TO STOP. MY DAUGHTER HAS BEEN WITH ME ON THREE OCCURANCE. I HAVE TAKEN IT TO THE DEALERSHIP TWICE AND THEY SAID THEY COULDN'T FIND ANYTHING WRONG. THIS IS A SAFETY ISSUE. SO FAR I'VE BEEN LUCKY AND NOT HIT THE REAR OF ANOTHER CAR, BUT I DON'T WANT TO TAKE A CHANCE.
WITHOUT CAUSE OR REASON, THE "BRAKE ACTUATOR" CIRCUIT MALFUNCTIONED CAUSING THE CAR TO LOSE CRUISE CONTROL, ABS, ANTI-SKID AND SPORADIC TAIL LIGHT PROBLEMS (LIGHTS WILL STAY ON OR COME ON WHEN THE CAR IS PARKED). I BROUGHT IT TO MY TOYOTA DEALER AND THEY WERE UNABLE TO IDENTIFY THE PROBLEM. THE NEXT DAY, THE ROADS WERE SNOW COVERED AND THE BRAKES LOCKED UP APPROACHING AN INTERSECTION CAUSING THE CAR TO SKID INTO ANOTHER VEHICLE. WHEN I BROUGHT THE CAR BACK TO THE DEALER, AFTER THE BODY WAS REPAIRED, THEY LOCATED THE PROBLEM AND SAID IT WOULD COST $2900 TO REPLACE. BEFORE I WAS ABLE TO HAVE THE VEHICLE REPAIRED, THE WIRING CAUGHT FIRE, PROBABLY BECAUSE THE TAILLIGHTS CAME ON BY THEMSELVES DURING THE NIGHT, AS THEY OCCASIONALLY DID (WITHOUT A KEY IN THE IGNITION). THE CAR WAS TOWED TO THE TOYOTA DEALER WHO ESTIMATES UPWARDS OF $10,000 FOR REPAIRS TO THE COMPUTER AND WIRING SYSTEM. THE TOYOTA SERVICE MANAGER RECOMMENDS WE SELL THE $19,000 CAR FOR "WHOLESALE." INSURANCE WILL NOT COVER THE DAMAGE AS IT WAS A MECHANICAL MALFUNCTION. TOYOTA CORP. IN CALIFORNIA, CONSUMER EXPERIENCES DEPARTMENT, WILL NOT GIVE ME ANY HELP AS THE CAR IS 21,000 OVER THE 36,000 MILE WARRANTY. HOW OR WHERE CAN I GET A LAWYER OR CONSUMER ADVOCATE TO HELP ME?

I WAS WAITING AT A STOP LIGHT ON NORRISTOWN ROAD TO CROSS BETHLEHEM PIKE IN SPRINGHOUSE, PA AT APPROXIMATELY 5 PM ON 5/22/15. I WAS FIRST IN LINE TO CROSS THE INTERSECTION IN THE DIRECTION OF PICKING UP 309 SOUTH IN ABOUT 1/2 MILE. I HAD BEEN WAITING AT THE LIGHT FOR A MINUTE OR TWO WHEN MY CAR SURGED FORWARD. I PUT BOTH FEET ON THE BRAKE BUT I WAS MOVING INTO THE INTERSECTION NONETHELESS. THERE WAS SPARSE TRAFFIC AND IT WAS SAFE FOR ME TO TURN RIGHT ONTO BETHLETHEM PIKE. ALMOST AS SOON AS I MADE THE TURN ONTO BETHLEHEM PIKE THE SURGE DISSIPATED AND THE ENGINE PERFORMANCE RETURNED TO NORMAL. THE WHOLE INCIDENT TOOK ABOUT 30 SECONDS TO A MINUTE. IT WAS PROBABLY ILL-ADVISED, BUT I CONTINUED TO DRIVE TO MY DESTINATION, HOME. UPON REACHING HOME AND THINKING ABOUT WHAT HAD HAPPENNED I HAD THE CAR TOWED THE NEXT MORNING (YESTERDAY) TO A DEALERSHIP FOR REPAIR. THE CAR AT THIS MOMENT IS THERE AWAITING REPAIR, I HAD TO ESTIMATE THE NUMBER OF MILES ON THE ODOMETER. ON THE FIRST LEG OF MY ERRAND ON THAT DAY (FRIDAY 5/22), I DROVE NORTH ON RT 309. I NOTICED THAT AT FREEWAY SPEED THE TACHOMETER WAS IN THE NEIGHBORHOOD OF 3000 RPM. I HAD DONE A LONG TRIP WITH THIS CAR THE PREVIOUS SUNDAY, 5/17, AND AT CRUISING SPEED THE TACHOMETER WAS IN THE VICINITY OF 2100 RPM.
THE DRIVERS SIDE SUNVISOR STARTED TO DROP DOWN AND WILL NOT STAY IN THE UP POSITION. WE (HUNBAND AND SONS) HAVE ALL HIT OUR HEADS ON THE ON THIS DANGLING SUNVISOR SEVERAL TIMES. THE FRAME OF THIS SUNVISOR IS VERY HARD AND IT ALMOST GAVE ME A HEADACHE. MY A THE DRIVES HEAD IS HIT HARD ENOUGH DURING A SUDDEN STOP THIS THING COULD KNOCK THE DRIVE OUT COLD. GOOD AFTERNOON SIR/MA'AM, I AM A GOVERNMENT EMPLOYEE AT DOVER AFB. THIS VEHICLE IS NOT MINE; HOWEVER, IT WAS ONE I HAD INTERACTION TODAY. AS I WALKED TO MY OWN VEHICLE I WAS SUMMONED TO A 2012-2013 TOYOTA RAV4 TO A DISTRESSED MOTHER. DRIVING ON ROAD, HEADED TOWARDS HIGHWAY. LIGHT WAS GREEN AND I CONTINUED TO FORWARD TO GET ONTO HIGHWAY. A DRIVER A COUPLE CARS AHEAD STOPPED SUDDENLY BEFORE ENTERING ONRAMP TO HIGHWAY AND I SLAMMED INTO THE BACK END OF THE TRUCK IN FRONT OF ME. THE TRUCK WAS JACKED UP SOME AND I HIT A STEEL BUMPER, WITH A TRAILER HITCH. MY CAR WAS NOT DRIVABLE, FRONT RIGHT SIDE RECEIVED MOST OF THE DAMAGE, BUT MY CAR WAS DETERMINED TOTALED AS IT HAD MORE THAN $14,000 DAMAGE. THE ENGINE WASN'T DAMAGED, THE WINDSHIELD FLUID WAS STILL IN THE CONTAINER, THE BATTERY WASN'T DAMAGED, YET THE CAR WAS TOTALED. THE DRIVER OF THE OTHER CAR TOOK OFF AFTER I HIT HIM. IN THE QUESTION ABOVE, I CHECKED THE AFFECTED PARTS AS THE AIRBAGS, BECAUSE THAT'S WHERE MY CONCERN IS. THE AIRBAGS DIDN'T DEPLOY AND I RECEIVED WHIPLASH WITH A TORN LIGAMENT IN MY NECK. I HAVE ADDITIONAL INJURIES ON MY KNEE, AS MY KNEE SLAMMED INTO SOMETHING, STEERING WHEEL, BOTTOM OF DASHBOARD OR SOMETHING. ALL ARE CONSIDERED MINOR INJURIES, AS I WALKED AWAY. ON THURSDAY 18 DECEMBER, MY WIFE AND I LEFT SPRINGFIELD, MISSOURI TO DALLAS, TEXAS TO VISIT OUR FAMILY FOR THE CHRISTMAS HOLIDAY. WE WERE DRIVING OUR RECENTLY PURCHASED 2012 RAV4 WITH APPROXIMATELY 33,000 MILES WHICH WE PURCHASED IN LATE SEPTEMBER 2014. WE STOPPED SEVERAL TIMES DURING OUR TRIP TO REFUEL AND ALLOW MY WIFE, WHO IS 6 MONTH PREGNANT, A CHANCE TO STRETCH HER LEGS. FOR MUCH OF THE TRIP, I USED CRUISE CONTROL WHEN POSSIBLE AT VARYING SPEEDS FROM 65 MPH (IN MISSOURI) TO 75 MPH (IN PARTS OF OKLAHOMA). AT APPROXIMATELY 4:30 PM, WHILE DRIVING THROUGH TUSHKA, OKLAHOMA WE APPROACHED A STOPLIGHT WHERE THE SPEED LIMIT DROPPED FROM 65 TO 45 MPH. THE LIGHT WAS RED, A SEMI WAS STOPPED AT THE LIGHT, AND I ATTEMPTED TO APPLY THE CAR'S BRAKES. AS I DID SO, THE CAR FELT AS IF IT WAS FIGHTING MY ATTEMPTS TO STOP IT. I HAD TO KEEP BOTH HANDS ON THE WHEEL, AND I DO NOT KNOW IF THE CRUISE CONTROL DISENGAGED. AS I APPROACHED THE SEMI, I PRESSED HARDER ON THE BRAKES IN ORDER TO STOP MY VEHICLE. MY INTENT WAS TO RE-START THE CAR AND SAFELY PULL OVER AFTER WE PROCEEDED THROUGH THE INTERSECTION. HOWEVER, AT THIS POINT THE LIGHT TURNED GREEN AND, AS I RESTARTED THE CAR, WE WERE HIT FROM BEHIND BY ANOTHER VEHICLE. I WAS DRIVING IN THE PARKING GARAGE AT WORK AND THE YELLOW EXCLAMATION LIGHT APPEARED ON THE DASH WITH A MESSAGE THAT SAYS "CHECK EV SYSTEM. HAVE YOU VEHICLE CHECKED BY A DEALERSHIP IMMEDIATELY." I WAS ABLE TO STOP AND THE CAR STARTED AFTER SEVERAL ATTEMPTS. THE DEALERSHIP REPLACED THE ECU. *TR
WHILE DRIVING 35 ON A LOCAL ROAD, THE VEHICLE LOST ALL POWER TO THE PROPULSION SYSTEM. POWER STEERING AND BRAKES WORKED AS I WAS LUCKY TO BE ABLE TO COAST SAFELY TO A TURNOUT. THE VEHICLE WARNING LIGHT SAYING "CHECK EV SYSTEM, HAVE DEALER CHECK IMMEDIATELY" WAS ILLUMINATED ON THE INFORMATION DISPLAY. THIS IS A COMMON PROBLEM FOR THE RAV4 EV AS THE TOYOTA AND TESLA PARTS HAVE TROUBLE MAINTAINING COMMUNICATION WITH EACH OTHER WITHIN A PART CALLED THE GATEWAY ECM. MY CAR WAS SERVICED BY THE DEALER AFTER THIS INCIDENT, WHERE THEY UPDATED THE FIRMWARE LOADED ON THE GATEWAY ECM.

ON SEPTEMBER 6, 2014 AT 4:20PM WHILE PULLING FORWARD INTO A PARKING SPACE THE CAR HAD AN UNINTENDED ACCELERATION. THIS IS THE THIRD TIME WITH THE EXACT SAME CIRCUMSTANCES. I PULLED FORWARD INTO A PARKING SPACE AND STOPPED. I PLACED THE CAR INTO REVERSE WITH MY FOOT ON THE BRAKE TO RE-ADJUST THE POSITION OF THE VEHICLE IN THE SPACE. BACKED OUT WITHOUT APPLYING FOOT TO ACCELERATOR. I APPLIED THE BRAKED WHEN CAR WAS OUT OF THE SPACE. WITH FOOT ON BRAKE THEN SHIFTED INTO DRIVE AND THE CAR IMMEDIATELY ACCELERATED TO 4,000 RPMS. THE WEATHER WAS CLEAR, 84 DEGREES, 77% HUMIDITY, DRY PAVEMENT, NO ALL WEATHER FLOOR MATS. THE CAR HAD RECENTLY HAD AN ECU PROGRAMMING UPDATE AT 17,000 MILES, WHEN AT TOYOTA FOR AN OIL CHANGE. TRYING TO USE THE 'HANDS FREE' FEATURE OF THE BLUETOOTH FEATURE IN THE RAV4 TOYOTA, I RAN INTO A CURB TRYING TO FIND A TELEPHONE NUMBER IN THE PHONE BOOK. I HAD TO PURCHASE A NEW TIRE FOR OVER $200. ON JANUARY 25 2014 I WAS DRIVING IN S SNOWY AND ICY CONDITIONS. I WAS ONLY DRIVING ABOUT 20 MILES AN HOUR. WHEN BRAKING, TURNING CORNERS, OR GENERALLY DRIVING ON THE BAD ROADS, THE ELECTRONIC STABILITY CONTROL, ANTI-LOCK BRAKES, AND TRACTION CONTROL FEATURES OF MY RAV4 DID NOT WORK, CAUSING THE CAR TO SLIDE ALL OVER THE ROAD, ALMOST CAUSING ME TO HIT ANOTHER VEHICLE. THIS HAS CONTINUED THROUGHOUT THE WINTER; THE LIGHT FOR STABILITY CONTROL ON THE DASHBOARD GOES ON, BUT THE ANTI LOCK BRAKES, ESC, AND TRACTION CONTROL DO NOT WORK AND THE RAV4 SLIDES ALL OVER. GIVEN THE BAD WEATHER WE ARE EXPERIENCING THIS WINTER, IT HAS CAUSED ME TO NOT DRIVE THE VEHICLE, WHICH IS A MAJOR INCONVENIENCE FOR OUR FAMILY. I TOOK THE RAV4 INTO THE LOCAL TOYOTA DEALER FOR THIS ISSUE ON JANUARY 31, 2014. THEY SAID THEY COULD NOT REPRODUCE THE PROBLEM IN THEIR SHOP SO DISMISSED IT WITH NO ACTION AND NO REPAIR I READ A COUPLE OF WEEKS OTHER THAT TOYOTA HAS RECALLED RAV4'S FOR EXACTLY THIS ISSUE; HOWEVER, WHEN I CALLED THE DEALER ABOUT THE RECALL, HE STILL INSISTS THAT THERE IS NOTHING WRONG WITH MY RAV4. SO FAR I HAVE GOTTEN NOTHING FROM TOYOTA ABOUT THIS RECALL, AND I DON'T FEEL SAFE DRIVING THE RAV4 THIS WINTER, A MAJOR INCONVENIENCE IN COMMUTING TO WORK. I FEEL MY RAV4 IS UNDRIVABLE THIS WINTER. *TR
I DROVE VEHICLE IN THE MORNING, NO INDICATIONS OF ANY STEERING PROBLEMS. DURING MY DRIVE IN THE LATE AFTERNOON, THE POWER STEERING ASSIST STOPPED WORKING. I WAS STOPPED AT A LEFT TURN LANE AND HEARD A FAINT "CLICK" SOUND FROM BELOW THE STEERING WHEEL AREA AT AROUND KNEE LEVEL. THIS SOUND WAS NOT THAT OF A "TURN SIGNAL SWITCH" CLICK, IT WAS JUST A "CLICK". THE ENGINE STAYED RUNNING, THE EPS WARNING LIGHT CAME ON, THE STEERING WHEEL WAS VERY HARD TO TURN (I HAVE CARPEL TUNNEL IN MY RIGHT HAND), WITH ASSISTANCE FROM MY PASSENGER WE WERE ABLE TO COMPLETE THE TURN. WE DROVE VEHICLE HOME WITHOUT POWER STEERING AND PARKED IT. MY BROTHER-IN-LAW CAME THAT EVENING TO INSPECT FOR FAILURE. VEHICLE STARTED UP AND POWER STEERING WORKED WITHOUT PROBLEMS. SINCE NO EPS WARNING LIGHT CAME ON, HE VOLUNTEERED TO TAKE VEHICLE HOME TO TEST DRIVE UNTIL PROBLEM RECURRED. THE DEALER TOLD HIM TO DRIVE THE VEHICLE TO THEM TO HAVE PROBLEM EVALUATED. HE HAS PUT 500 MILES ON THE ODOMETER SO FAR AND PROBLEM HAS NOT RECURRED. HE SAID THE DEALER WILL JUST GIVE ME THE RUN AROUND IF NO "HARD CODE" IS SET AND CONDITION OF NO" POWER ASSIST" IS PRESENT. I AM AFRAID OF DRIVING THIS VEHICLE NOW BECAUSE OF THIS PROBLEM. *TR

APPROACHING STOP LIGHT AND SLOWING DOWN TO 35 MPH. STEPPED ON BRAKE WITH RIGHT FOOT AND ENGINE WENT FROM 2000 TO 3000 RPM WITHOUT ANYTHING AGAINST ACCELERATOR. STEPPED ON BRAKE AS HARD AS POSSIBLE, BUT COULD NOT GET VEHICLE TO SLOW DOWN UNTIL WELL PAST THE STOP LIGHT. THIS EXACT SAME THING HAPPENED AGAIN THREE DAYS LATER ON 2/14/14. NOW IT IS POSSIBLE THAT THIS COULD HAD HAPPEN ON THE ROAD AND I'M JUST WONDERING IF THIS HAS HAPPEN TO OTHER TOYOTA VEHICLES... I HAD JUST GOTTEN ON A 6 LANE EXPRESSWAY WITH A 45 MPH SPEED LIMIT. HANDS AT TEN AND TWO - JUST WATCHING THE ROAD GOING ABOUT 40 MPH IN THE FAR LEFT LANE SO PEOPLE COULD TURN ON THE ROAD. I HAD BEEN HAVING A CLUNKING SOUND WHEN I WOULD PULL OUT OF MY DRIVEWAY FOR AWHILE BUT DIDN'T WANT TO GO BACK TO THE DEALER BECAUSE I HAD JUST GOTTEN A 10,000 MILE OIL CHANGE AND AFTER REMOVING A TIRE WITH A NAIL IN IT FOUND THEY HAD TO ORDER THE TIRE, WHICH TOOK 3 WEEKS TO GET (APPARENTLY RUN FLAT TIRES ARE STILL IN JAPAN,,) SO, I BACK TO MY NEAR DEATH EXPERIENCE- I WAS DRIVING SLOWER THAN THE SPEED LIMIT DOWN THE ROAD WHEN I LOST ALL STEERING IN MY VEHICLE AND I WAS HEADED INTO ONCOMING TRAFFIC. I HIT THE TALL CEMENT MEDIAN AND I PULLED AS HARD AS I COULD BACK TO THE RIGHT. AFTER I HIT I HAD A LITTLE CONTROL AGAIN. 11 OCT 12 - I BOUGHT A 2012 RAV4. 24 OCT 12 - I STOPPED AT A STOP SIGN. AS I APPLIED THE BRAKE THE CAR ACCELERATED. I PUSHED THE BRAKE HARD AND THE CAR DECELERATED. A FEW MINUTES LATER I APPROACHED A STOP SIGN AT MY HOUSE. WHEN I APPLIED THE BRAKE THE CAR ACCELERATED. BEFORE I COULD PUSH HARDER ON THE BRAKE THE CAR RAN THE STOP SIGN AND THEN DECELERATED. I CALLED TOYOTA CUSTOMER SERVICE. I EXPLAINED WHAT HAPPENED WITH THE ACCELERATION ISSUES. I TOLD THE REP I LOOKED AT MY FOOT POSITION IN BETWEEN INCIDENTS AND THERE WAS NO WAY I COULD PRESS THE ACCELERATOR AND BRAKE PEDALS TOGETHER. THE REP TOLD ME THERE WAS NO RECALLS ON THIS YEAR RAV4. HE SAID TAKE THE CAR TO THE NEAREST TOYOTA DEALER. I TOOK THE CAR TO TOYOTA AND EXPLAINED EVERYTHING TO THEM. THE SERVICE MAN TOLD ME THAT THE FLOOR MAT BEING CAUGHT UP UNDERNEATH THE BRAKE WAS THE ISSUE. HE TOLD ME THAT THE REP FROM TOYOTA WOULD LOOK AT IT. 5 NOV 12 - TOYOTA CALLED TO SAY THE TOYOTA REP COULDN'T DUPLICATE THE PROBLEM. 6 NOV 12 - I WENT TO TOYOTA TO PICK UP MY CAR. I LEFT THE DEALERSHIP AND STOPPED THE CAR TO WAIT FOR TRAFFIC. AFTER SITTING WITH MY FOOT ON THE BRAKE FOR ABOUT 4-5 SECONDS, THE CAR BEGAN ACCELERATING ON ITS OWN. I FIRMLY PUSHED HARD ON THE BRAKE AND LOOKED AT MY FOOT TO ASSURE MYSELF I WASN'T NEAR THE GAS PEDAL. I VERIFIED THE FLOOR MAT WASN'T ON THE GAS PEDAL. I PLACED THE CAR IN NEUTRAL AS IT WAS REVVING UP AND NOTICED THE TACHOMETER WENT BETWEEN 6500 TO 7000 RPM'S (IN THE RED). WHEN TRAFFIC CLEARED FOR ME TO TURN, I MADE A TURN, THE CAR LUNGED FORWARD AND THE ACCELERATION DECELERATED. I DROVE THE CAR DIRECTLY TO TOYOTA AND EXPLAINED WHAT JUST HAPPENED. 7 NOV 12 I GOT A PHONE CALL FROM TOYOTA CUSTOMER SERVICE CORPORATE TELLING ME THAT I COULD PICK UP THE VEHICLE FROM THE TOYOTA DEALERSHIP. THEY COULD NOT DUPLICATE THE PROBLEM AND THEY SAID THE CAR WAS SAFE TO DRIVE. *TR

THE DRIVER SUSTAINED A LACERATION TO THE LEFT COLLAR BONE DUE TO THE TIGHTNESS OF THE SEAT BELT. A POLICE REPORT WAS FILED. THE VEHICLE WAS TOWED TO AN INDEPENDENT MECHANIC FOR DIAGNOSES AND REPAIR. THE MANUFACTURER WAS MADE AWARE OF THE PROBLEM. THE APPROXIMATE FAILURE MILEAGE WAS 11,000.
2012 TOYOTA RAV 4. CONSUMER WRITES IN REGARDS TO VEHICLE SUDDEN ACCELERATION. *SMD THE CONSUMER STATED ON SIX OCCASIONS, THE VEHICLE ACCELERATED, WHEN THE BRAKE WAS APPLIED. THE BRAKE PEDAL WAS ABOUT 2 1/2 INCHES AWAY FROM THE ACCELERATOR PEDAL. THE CONSUMER BELIEVED THE PEDALS WERE DESIGNED TOO CLOSE TOGETHER.
THE REAR SEAT SHAKES OVER ANY ROUGH SURFACE. IN WATCHING THE HEADREST FOR THE REAR SEAT IN THE REAR VIEW MIRROR WHILE DRIVING THERE IS ABOUT A 2-INCH SIDEWAYS TRAVEL. HAVE TRIED THE SEAT IN VARIOUS POSITIONS AND THE PROBLEM REMAINS. BY ROUGH I DON'T MEAN POTHOLES...THINGS LIKE CHANGING LANES OVER REFLECTOR BUTTONS, FREEWAYS WITH RAIN GROOVES, THINGS LIKE THAT. MY 12 YEAR OLD JEEP GRAND CHEROKEE'S REAR SEAT DOESN'T SHAKE AT ALL IN THESE CONDITIONS, BUT THIS RAV4 REAR SEAT MOST CERTAINLY DOES. THE SHAKING IS SO BAD SOMETIMES THAT IT RATTLES. I AM VERY CONCERNED ABOUT AN INFANT CAR SEAT STRAPPED INTO THIS BACK SEAT AS THE INFANT MY BE EXPOSED TO EXCESSIVE SHAKING OR VIBRATION. I TOOK IT TO THE DEALERSHIP (TOYOTA CARLSBAD, CARLSBAD CA) ON OCTOBER 7TH 2013 AND THEY VERIFIED THE ISSUE. RATHER THAN TAKE STEPS TO CORRECT IT THEY TESTED ANOTHER RAV4 IN THE SAME WAY AND FOUND THAT IT TOO HAD THE SAME CONDITION AS MINE. BASED ON THIS THEY TELL ME THAT IT IS A 'NORMAL CONDITION'. I BELIEVE IT IS A DESIGN FLAW. NEVER HAVE I SEEN A CAR WITH A SEAT SHAKE SUCH AS THIS. IN TESTING FURTHER I FIND THAT IF SOMEONE SITS IN THE FRONT PASSENGER SEAT THE REAR SEAT SHAKING SEEMS TO REDUCE. IF SOMEONE SITS IN THE REAR SEAT AND THEY WEIGH 150 POUNDS OR SO THERE IS A MINOR VIBRATION FELT AND THE SHAKING IS ALMOST NON-EXISTENT. HOWEVER, IF ONLY THE DRIVER IS IN THE CAR THE SHAKING RESUMES OR ANY LESS-THAN-PERFECT ROAD CONDITION. I FEEL AS THOUGH TOYOTA NEEDS TO ADDRESS THIS ISSUE PRIMARILY FOR THE RISK OF INJURY TO INFANTS STRAPPED IN THE SEAT AS WELL AS COCKPIT NOISE (RATTLE). IT IS A FLAW IN THE DESIGN OF THE VEHICLE TO BE SURE. *TR
I AM FILING A COMPLAINT ABOUT SPONGY BRAKES BEHAVIOR IN A 2012 TOYOTA RAV4. THE PROBLEM I WAS EXPERIENCING WERE THE BRAKE PEDAL TRAVELING TO THE FLOOR AND REDUCED STOPPING POWER DURING APPLICATION. THE SPONGY BEHAVIOR OF THE BRAKE PEDAL WAS ALSO PRESENT WITH ENGINE OFF. I REPORT THIS EVENT TO TWO SEPARATE TOYOTA DEALERSHIPS WHICH TECHNICIANS CLAIMED THIS IS A NORMAL FUNCTION AND THIS IS A SOFT PEDAL DESIGN (ACCORDING TO THE FIRST DEALERSHIP). THE SECOND DEALERSHIP SHOWED ME ANOTHER SIMILAR VEHICLE (2011 TOYOTA RAV4) AND IT ALMOST DID THE SAME THING (ENGINE OFF - PEDAL WAS STILL SPONGY BUT WAS LESS THAN MY VEHICLE). SINCE BEING A ASE MASTER TECHNICIAN BEFORE BECOMING AN MECHANICAL ENGINEER, I BLEED THE BRAKES WHICH ONLY HELPED A LITTLE AND THERE WERE NO FLUID LEAKS. FINALLY, I RESEARCHED THAT THE ABS PUMP MIGHT NEED TO BE ACTIVATED USING A SCANNER TO CYCLE THE ACTUATORS THAT MIGHT HOLD AIR TRAPPED INSIDE. I TOOK THE CAR TO MY PREVIOUS PLACE ON EMPLOYMENT WHERE I CONSULTED THIS ISSUE WITH THE SHOP MANAGER IN WHICH HE AGREED THAT THIS IS A POSSIBILITY. THE SHOP MANAGER PERFORMED THE ABS PUMP CYCLING PROCESS WITH THE SCANNER AND THE BRAKE PEDAL BECAME HARD AND FIRM LIKE IT SUPPOSE TO BE (PROCEDURE TIME: 5 MINUTES!). I AM VERY DISAPPOINTED WITH TOYOTA TRAINED TECHNICIANS AND ANY GOOD TECHNICIAN KNOWS THERE IS NO SUCH THING AS SOFT PEDAL DESIGN ESPECIALLY WITH A VACUUM BRAKE BOOSTER WITH ENGINE OFF (HYDRAULIC FLUID IS ONLY SLIGHTLY COMPRESSIBLE!). ONLY VEHICLES WITH HYDRAULIC BRAKE BOOSTER CAN EXPERIENCE A BRAKE PEDAL FEELING SPONGY SINCE THE BOOSTER WILL CONTINUE TO FUNCTION AFTER ENGINE HAD BEEN TURNED OFF (APPROX:10 ' 20 BRAKE PEDAL APPLICATIONS ' DIESEL & VERY FEW GAS VEHICLES). IT SEEMS TO ME TOYOTA IS SKIPPING A STEP IN THERE MANUFACTURING PROCESS. *TR
ON THREE OCCASIONS DURING 2013, THE CAR ACCELERATED AND JUMPED FORWARD AS THE BRAKE WAS BEING APPLIED TO STOP THE CAR. ON SEPTEMBER 5, 2013 THE ACCELERATION AND MOVEMENT FORWARD WAS MORE PRONOUNCED AND WAS ACCOMPANIED BY A NOTICEABLE SOUND OF THE ENGINE. THIS OCCURRED AS THE BRAKE WAS BEING SLOWLY APPLIED DURING AN APPROACH TO AN INTERSECTION. THERE WAS NO MOVEMENT OR INVOLVEMENT OF THE FLOOR MAT ON THE DRIVER?S SIDE AND NO POSSIBILITY THAT THE DRIVER'S FOOT COULD HAVE HIT THE ACCELERATOR AT THE SAME TIME AS HIS FOOT WAS PLACED ON THE BRAKE. WE REQUESTED THAT THE CAR BE EXAMINED BY THE TOYOTA DEALER IN ORDER TO DETERMINE THE NATURE OF THE DEFECT AND THE CORRECTIVE ACTION NEEDED. THE DEALER REPORTED THAT IT COULD NOT IDENTIFY ANY PROBLEM. *TR
I WAS SLOWLY MANEUVERING INTO A PARKING SPACE WHEN THE CAR REVVED UP, JUMPED OVER THE CONCRETE BARRIER, OVER THE CURB AND SIDEWALK. I WAS HEADED DIRECTLY TOWARD A BUILDING AND A LARGE PLATE GLASS WINDOW. THE BRAKES DID NOT STOP THE CAR, AND FELT SQUISHY WHEN I PUSHED THEM ALL THE WAY TO THE FLOOR. I SWERVED TO THE RIGHT, TURNED OFF THE KEY AND HIT A PARKED CAR IN ORDER TO STOP. I ALSO WANT TO MENTION THAT THE TIRE PRESSURE GAUGE LIGHT WOULD COME ON ANY TIME I WAS DRIVING ON THE HIGHWAY AT SPEEDS OVER 50 MPH. *TR
ACCELERATING FROM A STOP LIGHT, THE CAR LOST POWER. THE ELECTRIC MOTOR STOPPED AND THE CHECK EV SYSTEM WARNING CAME ON. THERE WAS NO ENGINE POWER, POWER STEERING OR POWER BRAKES. THE CAR WAS STOPPED IN THE MIDDLE OF THE STREET. *TR

THE TECHNICIAN STATED THAT IT WAS NORMAL FOR A NEW VEHICLE TO MAKE NOISES BUT PROCEEDED TO REPLACE THE BRAKES PAD AND SHIM KIT. ADDITIONALLY, ALL OF THE BRAKE COMPONENTS WERE LUBRICATED HOWEVER, THE FAILURE RECURRED. THE CONTACT STATED THAT WHILE DRIVING 40 MPH, SHE ATTEMPTED TO APPLY THE BRAKES AND THEY FAILED TO RESPOND. AS A RESULT, SHE CRASHED INTO THE REAR OF ANOTHER VEHICLE. THE CONTACT SUSTAINED WHIPLASH AND MINOR BURNS CAUSED BY THE SEAT BELT AND DEPLOYMENT OF THE AIR BAG. A POLICE REPORT WAS FILED. THE DRIVER OF THE SECOND VEHICLE WAS ALSO INJURED AND TRANSPORTED TO HOSPITAL. THE VEHICLE WAS TAKEN TO THE DEALER AND WAS NOT REPAIRED OR INSPECTED. THE MANUFACTURER WAS MADE AWARE OF THE FAILURE. THE VIN WAS NOT AVAILABLE. THE FAILURE MILEAGE WAS 500 AND THE CURRENT MILEAGE WAS 17,000.

THE LATEST INCIDENT WAS THE 8TH TIME THIS HAS OCCURRED SINCE PURCHASED NEW IN SEPT. 2012. AFTER HIGHWAY DRIVING APPROACHED A TRAFFIC LIGHT AND WHEN BRAKE APPLIED CAR REVVED UP AND LURCHED FORWARD. SINCE NOTHING STOPS THIS EXCEPT TURNING OFF IGNITION THIS WAS DONE AGAIN AND CAR RESTARTED AND NO FURTHER INCIDENT. CONTACTED DEALERSHIP AND AFTER HAVING CAR THERE FOR 2 DAYS WAS TOLD NO REPLICATION COULD BE MADE. I CONTACTED THE TOYOTA OFFICE IN CALIFORNIA AND AFTER MORE DELAYS HAVE NOT BEEN GIVEN A RENTAL. CAR IS STILL AT DEALERSHIP AND THE SERVICE MANAGER SAID HE IS DOING NOTHING UNTIL HE HEARS FROM CALIFORNIA THAT MAY APPROVE SUPER TECH TO LOOK AT CAR. ON A SUNNY HOT DAY VEHICLE WAS PARKED FOR APPROX 20 OUTSIDE. ENTERED THE VEHICLE ONCE NO MISHAP. UPON SECOND ENTRY "3 MINUTES APART" WHEN DOOR WAS CLOSED REAR WINDOW JUST FELL APART. I FOUND GLASS ON THE GROUND OUTSIDE OF THE VEHICLE AND THE REMAINING GLASS IN THE VEHICLE. WHAT APPEARS TO HAVE HAPPEN IS THE REAR WINDOW EXPLODED. NOT AWARE OF ANY PRIOR DAMAGE TO THE WINDOW. TOYOTA WAS GIVEN TWO ATTEMPTS TO REVIEW THE GRIEVANCE ON EACH OCCASION CLAIMING OWNER FAULT OR RESPONSIBILITY. *TR
2012 TOYOTA RAV4 REAR CARGO DOOR WINDOW SHATTERED, AND GLASS FELL INSIDE VEHICLE AND OUTSIDE VEHICLE. THIS OCCURED WHILE CAR WAS PARKED, AND THE DRIVERS DOOR WAS BEING CLOSED. VEHICLE WAS SLOWING DOWN FROM ORIGINAL SPEED OF 45MPH APPROACHING A TRAFFIC LIGHT. AFTER APPLYING THE BRAKE THE VEHICLE'S ENGINE STARTED TO REV UP AND ACCELERATE WITH FOOT ON THE BRAKE. THE HARDER THE BRAKE PEDAL DEPRESSED THE HIGHER THE RPMS AND FOOT WAS NOT TOUCHING THE GAS PEDAL. AFTER A FEW SECONDS AND BRAKE PRESSED AS HARD AS POSSIBLE THE ENGINE QUIT RUNNING AND THE CAR WAS TOTALLY SHUT OFF AFTER JOLTING FORWARD. RAN FINE FOR THE 4 MILES OR SO TO THE HARDWARE. UPON STARTING THE CAR, AFTER LEAVING THE HARDWARE, IT REVVED UP TO A VERY HIGH RPM THEN DROPPED BACK TO NORMAL. DRIVE BACK HOME WAS OK. ON THE WAY TO THE TOYOTA DEALER ON JUNE 4 I COULD WATCH RPMS FLUCTUATE IN AN ABNORMAL MANNER AFTER HITTING THE BRAKE FOR A STOP AT TRAFFIC LIGHT. WHILE AT THE DEALER I FILED A CASE WITH TOYOTA. AFTER OWNING MY NEW 2012 RAV 4 FOR ONLY 3 DAYS, I VISITED A FRIEND'S HOUSE AND ATTEMPTED TO PARK MY CAR ON A SLIGHTLY SLOPED DRIVEWAY. IN FRONT OF ME, I SAW A PILEUP, BOTH LANES OF TRAFFIC WERE BLOCKED. I TAPPED MY BRAKES TO SLOW DOWN BUT BECAUSE OF THE ICE, I DID NOT STOP OR SLOW DOWN. AS SOON AS I TOOK MY FOOT OFF THE BRAKE THE CAR SURGED FORWARD LIKE THE GAS PEDAL WAS STUCK. THIS HAPPENED ABOUT ONE QUARTER MILE FROM MY HOUSE. I MADE IT HOME SAFELY. AFTER I GOT IN THE DRIVEWAY, I TURNED OFF THE ENGINE AND RESTARTED THE CAR BACK UP AGAIN. I DROVE IT UP AND DOWN THE DRIVEWAY A FEW TIMES AND IT WAS FINE. MY HUSBAND CHECKED THE FLOOR MAT AND IT WAS IN PLACE. VERY SCARY! *TR
DRIVER SIDE WINDOW SPONTANEOUSLY EXPLODED AND SHATTERED, SENDING GLASS PARTICLES THROUGHOUT THE PASSENGER COMPARTMENT. DRIVER SUSTAINED SMALL CUT TO LEFT MIDDLE FINGER. ROAD CONDITIONS WERE SUNNY AND DRY WITHOUT ROAD HAZARDS. NO OTHER VEHICLES IN VICINITY. NO GRAVEL ON ROAD. WINDOW APPEARED TO BE INTACT PRIOR TO THIS SPONTANEOUS INCIDENT. *TR

I PARKED MY CAR IN RESIDENTIAL NEIGHBORHOOD AND PROCEEDED TO CROSS STREET WITH MY CAR PARKED BEHIND ME. THANKFULLY A MAN FACING CAR YELLED "YOUR CAR!" MY NEW CAR WAS ROLLING DOWN THE STREET BACKWARDS, GAINING SPEED DOWN THE INCLINE, HEADED FOR CURB, SIDEWALK AND HUGE TREE. I CHASED THE CAR DOWN THE STREET, GOT DOOR OPEN, JUMPED INSIDE VEHICLE AS IT WAS MOVING AND SLAMMED ON BRAKE, INJURING MY BACK AND SHOULDER IN THE PROCESS. THE PUSH-BUTTON START ALLOWED CAR TO "FREE-WHEEL" OUT OF CONTROL WHEN TURNED OFF, LEFT IN DRIVE, AND KEY LEFT IN CAR. I AM IN AND OUT OF CAR MANY TIMES A DAY. NO OTHER CAR I HAVE EVER OWNED WILL ALLOW ME, WITHOUT WARNING, TO LEAVE CAR IF LEFT IN "DRIVE". I WAS NOT INFORMED BY TOYOTA THAT PUSH-BUTTON IGNITION WOULD ALLOW MY CAR TO BECOME A POTENTIAL WEAPON, FREE WHEELING, IF LEFT IN DRIVE AND UNATTENDED. I WOULD LIKE THIS SITUATION TO BE REMEDIED TO SAVE LIVES AND PROPERTY DAMAGE. EVEN AFTER GOING THROUGH THIS FRIGHTENING EXPERIENCE, I CANNOT BE SURE IT WON'T HAPPEN AGAIN WITHOUT AN EFFECTIVE WARNING SYSTEM. *TR
I PULLED INTO A LEFT TURN LANE TO MAKE A TURN. AS I WAS WAITING FOR ONCOMING TRAFFIC TO CLEAR THE CAR SUDDENLY SURGED FORWARD. I SLAMMED ON THE BRAKES. THAT STOPPED THE CAR. FORTUNATELY THERE WASN'T A CAR IN FRONT OF ME, OTHERWISE I WOULD HAVE HAD AN ACCIDENT. I WAS RIDING WITH MY COUSIN AND WE WERE TRAVELING ABOUT 25 MILES AN HOUR ON A BUSY STREET. THE CAR SURGED, MY COUSIN ASKED IF I HAD STEPPED ON THE GAS, I HADN'T. I IMMEDIATELY TOOK THE CAR TO THE TOYOTA DEALERSHIP. I TOLD THE TECH WHAT HAD HAPPENED, HE SAID THAT WAS IMPOSSIBLE. TOOK THE CAR FOR A SHORT DRIVE AND SAID THERE WAS NOTHING WRONG WITH THE CAR. IT CONTINUES TO HAPPEN PERIODICALLY. I HAVE ALSO NOTICED THIS WEEK THAT THE TRANSMISSION SEEMS TO LUG DOWN WHEN IT SHIFTS. I AM CONCERNED ABOUT THE OVERALL PERFORMANCE OF THIS CAR, NOT TO MENTION THE DEFECT IN THE PARALYZED WHITE PAINT. ONE TRIP THROUGH THE CAR WASH WITH NORMAL DIRT AND THE TOP LAYER OF PAINT CAME OFF IN ONE AREA. THE DEALERSHIP IS TO CALL ME WHEN THE REP COMES IN TO LOOK AT THE PAINT THAT WAS WEEKS AGO AND NO ONE HAS CALLED. *TR
I OPENED THE VEHICLE TRUNK TO PLACE A SMALL BOX INSIDE. WHEN I SHUT THE TRUNK DOOR, THE GLASS ON THE PASSENGER-SIDE, REAR QUARTER PANEL SHATTERED. THE LOCATION WAS PARTICULARLY TROUBLING BECAUSE THE WINDOW WAS LOCATED DIRECTLY BEHIND MY INFANT'S CAR SEAT. THERE WERE NO PRIOR SCRATCHES OR CRACKS ON THE PANE. ALSO, THERE WAS NOTHING ON OR NEAR THE GLASS THAT WOULD HAVE CONTRIBUTED TO THE INCIDENT. *TR
THIS IS A LEASED CAR. THIS DATE WAS THE FIRST EPISODE OF A RECURRING PROBLEM. EACH TIME IT TOOK PLACE ON A LOCAL STREET. EACH TIME AS THE DRIVER ( MY WIFE OR MYSELF ) WAS STEPPING ON THE BRAKE TO BRING THE CAR TO A FULL STOP THE CAR SUDDENLY STARTED TO ACCELERATE. THIS FIRST INCIDENT WAS NOTEWORTHY BUT WAS ATTRIBUTED TO THE NEWNESS OF THE VEHICLE. THE THREE INCIDENTS THAT FOLLOWED WERE CONSIDERABLY WORSE. WE CONTACTED THE DEALER FROM WHOM THE CAR IS LEASED AND WERE TOLD TO BRING THE CAR IN . WE BROUGHT THE CAR IN ON AUGUST 23 AND PICKED IT UP ON AUGUST 30. AT THAT TIME WE WERE TOLD THAT THE CAR HAD BEEN TESTED AND NOTHING WA WRONG WITH IT. IN OTHER WORDS, TOYOTA HAS NO EXPLANATION OF WHY THESE FOUR INCIDENTS TOOK PLACE. THIS VEHICLE WAS PURCHASED SO THAT WE COULD SAFELY TRANSPORT OUR GRANDCHILDREN.THE LACK OF RESOLUTION OF THE PROBLEM LEAVES US WITH AN UNSAFE VEHICLE THAT WE WOULD NEVER USE FOR THE ORIGINAL INTENDED PURPOSE.THE AUGUST 29, 2012, WALL STREET JOURNAL COLUMN 'ME AND MY CAR' HAS A LETTER DESCRIBING A VERY SIMILAR PROBLEM ON A TOYOTA HIGHLANDER THAT WAS BROUGHT TO THE DEALER TWICE. EACH TIME THE LETTER WRITER WAS TOLD WHAT SEEMS TO BE TOYOTA'S STOCK ANSWER, "NOTHING IS WRONG'. THE WSJ COLUMNIST RESPONDS THAT ' COMPLAINTS ABOUT THE ELECTRONIC SPEEDCONTROL SYSTEMS ON TOYOTA (AND OTHER BRANDS )ARE FREQUENT'. HE GOES ON TO SUGGEST THAT FIGURING THE CAUSE WILL TAKE SEVERAL VISITS TO TOYOTA. NO ADDITIONAL VISITS WERE SCHEDULED OR EVEN SUGGESTED BY TOYOTA. *TR
MY 2012 HAS DONE THIS VIRTUALLY FROM DAY 1. WHEN BRAKING, THERE IS A SUDDEN ACCELERATION FOR 1/4 TO 1/2 SECOND AT THE TIME THE TRANSMISSION DOWNSHIFTS. I ALSO NOTICE IT WHILE DRIVING AT HIGH SPEEDS AND BRAKING. IT IS MORE DANGEROUS AT SLOWER SPEEDS ESPECIALLY WHEN BRAKING AT A LIGHT WITH SOMEONE IN FRONT OF YOU. IT FEELS LIKE YOU ARE PRESSING BOTH THE BRAKE AND ACCELERATOR AT THE SAME TIME. IT DOES NOT DO THIS ALL THE TIME. MAYBE ONCE OR TWICE A DAY. I THINK IT HAS TO DO WITH HOW HARD YOU BRAKE AND WHAT SPEED YOU ARE GOING. IT IS MOST NOTICEABLE JUST BEFORE YOU COME TO A STOP. IT ALWAYS HAPPENS JUST AS THE TRANSMISSION AUTOMATICALLY DOWNSHIFTS AND MOST NOTICEABLE WHEN IT DOWNSHIFTS TO SECOND OR FIRST. I NOTICE IT WHEN BRAKING SAY FROM 65 TO 55 MPH AS WELL, BUT THE SUDDEN ACCELERATION AT THAT SPEED HAS NO OR MINIMAL AFFECT AND IS NOT THAT NOTICEABLE. *TR
MY REAR SUSPENSION PERIODICALLY THUMPS A FEW MINUTES AFTER I START DRIVING THE VEHICLE AND IS MOST NOTICEABLE ON A DIRT ROADS. IT SOUNDS LIKE A ROCK IS HITTING THE UNDERSIDE OF THE CAR, BUT I KNOW IT IS NOT A ROCK! IT ONLY DOES IT ONCE. IT'S LIKE THE SUSPENSION IS BINDING AND THEN RELEASES AFTER A CERTAIN DISTANCE DOWN THE ROAD. *TR
VEHICLE IS ONLY A FEW WEEKS OLD AND THE WINDSHIELD BEGAN DEVELOPING A CRACK FROM THE PASSANGER SIDE SPREADING TOWARDS THE CENTER OF AND DOWN TOWARDS THE BOTTOM OF THE PASSANGER SIDE. *TR
THE CAR WAS PARKED IN THE DRIVEWAY WITH THE DOORS LOCKED AND THE WINDOWS ROLLED UP. SOMETIME BETWEEN 10PM AND 5:30AM THERE WAS A FIRE THAT CREATED ENOUGH HEAT TO MELT THE COATING ON THE FRONT WINDSHIELD AND BLOW OUT THE FRONT WINDSHIELD, MELT THE CENTER OF THE DASH AND THE AREA BETWEEN THE VISORS WHERE THE CONTROLS FOR THE SUNROOF AND INTERIOR LIGHTS WERE. IT ALSO CAUSED A SHORT IN THE WIRING THAT RESULTED IN TURNING ON THE HEADLIGHTS EVEN THOUGH THE SWITCH WAS OFF. WE HAD TO DISCONNECT THE CAR BATTERY TO TURN OFF THE LIGHTS. TOYOTA DID AN INVESTIGATION AND DECIDED THAT THE AURORA BE ANY BABY ON THE DASH SPONTANEOUSLY COMBUSTED IN THE MIDDLE OF THE NIGHT CAUSING THE FIRE AND DAMAGE. SINCE THERE HAVE BEEN REPORTS OF OTHER TOYOTAS HAVING PROBLEMS WITH ELECTRICAL I THOUGHT YOU WOULD WANT TO KNOW ABOUT THIS CASE. *JS

THE CONTACT STATED THAT AFTER ATTEMPTING TO DRIVE IN REVERSE OUT OF A PARKING SPACE WITH THE BRAKE PEDAL ENGAGED, THE VEHICLE SUDDENLY ACCELERATED AND CRASH INTO A PARKED VEHICLE. A POLICE REPORT WAS FILED. THE DRIVER SUSTAINED BRUISES TO HER LEG. THE VEHICLE WAS TAKEN TO THE DEALER WHO WAS UNABLE TO DIAGNOSED THE FAILURE. THE MANUFACTURER WAS MADE AWARE OF THE FAILURE AND STATED THEY WOULD CONTACT HER AT A LATER DATE. THE VEHICLE WAS NOT REPAIRED. THE FAILURE AND CURRENT MILEAGE WAS 4,620. THE CONTACT APPLIED EXCESSIVE PRESSURE TO THE BRAKE PEDAL, BUT THE ENGINE CONTINUED TO REV. ANOTHER INDIVIDUAL OPENED THE DOORS AND TURNED OFF THE IGNITION. THE VEHICLE WAS THEN TOWED TO AN AUTHORIZED DEALER AND THEY WERE UNABLE TO DIAGNOSE THE FAILURE. THE VEHICLE WAS NOT REPAIRED. THE MANUFACTURER WAS MADE AWARE OF THE FAILURE AND OFFERED NO ASSISTANCE. THE FAILURE MILEAGE WAS 1,009.

Southeast Toyota Distributors, LLC (SET) is recalling certain models interspersed through model years 2009 through 2013 as follows: model year 2009-2012 Tacoma, 4Runner, Camry, Camry Hybrid, Prius, and RAV4; model year 2009-2010 Avalon, FJ Cruiser, and Highlander Hybrid; model year 2010-2013 model year Corolla, Sienna and Tundra; model year 2009-2013 Highlander and Venza; model year 2012 Prius V; and model year 2010-2012 Sequoia. During modification by SET to include accessories such as leather seat covers, seat heaters or headrest DVD systems, these vehicles may not have had the passenger seat occupant sensing system calibration tested. Without passing the calibration test, the occupant sensing system may not operate as designed.

Southeast Toyota is recalling certain model year 2008 and 2010-2013 Toyota Tundra, 2010-2012 Rav4, 2012 Toyota Sequoia, 2010-2011 Toyota Corolla, 2010-2011 Toyota Camry and Camry Hybrid, 2010-2013 Toyota Highlander and Highlander Hybrid, 2010-2013 Toyota FJ Cruiser, 2011 Toyota Land Cruiser, 2010-2013 Toyota Venza, 2010-2011 Toyota 4Runner, 2010-2013 Toyota Tacoma, 2011-2012 Toyota Sienna, 2012 Toyota Prius, 2013 Scion FR-S, 2011 Scion XD, 2011 Scion XB, and 2012 Scion TC vehicles. These vehicles were sold with labels that were outside the allowable one percent of accuracy of actual weight added. Thus, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) Number 110, "Tire Selection and Rims."

Toyota is recalling certain model year 2012 and 2013 Toyota Tacoma and Lexus RX350 vehicles and certain model year 2012 Toyota Rav4 vehicles. In the affected vehicles, the brake system contains a brake actuator that adjusts the fluid pressure of each wheel cylinder. An electrical component within the actuator may experience an increased resistance resulting in the illumination of various warning lights, including those for the Vehicle Stability Control (VSC) system, Antilock Brake System (ABS), and Traction Control system.
