blind spot warning on the lower left corner of the screen, which is covered by the right hand when driving. With the turn signal, it is impossible to the blind spot warning video without taking the right hand off from the steering wheel. A dangerous situation trying to turn the vehicle with one hand! Also Tesla model Y phantom breaks every time I used autopilot on interstate. Fortunately, no vehicles were behind me. An accident waiting to happen.

This is an ongoing issue. When using autopilot, I experience phantom braking, usually with large oncoming trucks on 2 lane highways. The problem is worse with logging trucks, but I've had it happen with vehicles as small as school buses. In the worst cases, the collision alarm sounds and the car slows up to 30mph immediately. I have this happen at least twice every morning on my 7 mile drive to town. I have videos that capture this event. Photos don't work

Summary

On February 1st, 2022 I was driving with my wife from the Naperville, IL area to Holland, MI. We were on I 94 headed east, approximately 20 miles west of Michigan City Indiana headed east. I was driving around 65 miles per hour. We were on a 4 lane highway- all the cars on those 4 lanes were headed in the same direction. There was a truck in front of us ( headed in the same direction ) that was located to the right of us in the lane to our right and it was about 60 or 70 feet in front of us. All of a sudden the car, that had been operating on Autopilot, braked suddenly. We went from 65 to almost 0 in a split second and if my wife and I weren't wearing seat belts we would have been very seriously injured. My neck suffered a whiplash and I'm still in pain several days later.

FSD, the design of FSD using the safety score is designed to defeat failure reporting to government agencies. If you allow the FSD system to drive on its own, it will crash, run into things, fail to stop, or stop at green lights constantly. What the driver score system does is force human beings to manage and mitigate its failures leaving a on paper result that the system is more safe than drivers driving on their own. This is 100% false, if you allow FSD to drive, it will fail almost 100% of the time in very quick succession. From lane change mistakes, to turning mistakes, not staying within its lane in a consistent manner, not putting the car into left turn lanes or stopping at green lights, which in itself is dangerous. The safety score system MUST be eliminated as it cherry picks those drivers willing to spend the time and energy to ANTICIPATE the failure/weakness of the software and mitigate its failure before it fails, thus providing the government with false "safety" numbers... Grab a car and drive it, it will not be long before it fails left and right!

I commonly will experience phantom breaking driving on two-lane roads, particlualy when meeting oncoming traffic. This occurs most commonly at the crest of a hill or when meeting trucks. It occurs nearly 100% of the time when meeting trucks at the crest of a hill. There are also random occurrences on flat roads with no other vehicles and no shadows or bridges. It also occurs on bridges and occasionally with shadows. Performance has been better on four lane roads with more traffic. It's as if the system needs to have other traffic to gauge itself. With no traffic on four lane roads, phantom braking will rarely occur. There is no single date of this occurrence. It happens ALL THE TIME.

The Tesla has been doing phanton breaking. For example on the highway using dynamic cruise control or autopilot. When a truck is merging to the right lane it will apply quite the break force. Almost got rear ended cause of this on the highway. Observed this a few times on drive from Ga to NJ. I also noticed going up and down hills with dynamic cruise control it will sometimes trigger the front collision warning or phanton break. This seems to be sporadic and doesn’t like one lane back roads. Last and it least I also noticed that using auto pilot the car forces you to use auto high beans. On the highway at night it will go off and on a lot to the point drivers on opposite side of the road would flash their lights to let me know about the high beans. The model teslas doesn’t use radar or laser sensors like previous year. It does seem to have a lot of impact cause I did not see these issues with my Lexus RX350 With dynamic cruise control, lane keep assist, auto high beans, and front collision warning.

When cruise control (TACC) enabled the car will brake on 2 lane undivided roads when oncoming vehicle is a semi or other large vehicle. Vehicle may suddenly slow 5-15mph causing unsafe condition if being followed closely. Problem is easily reproducible on any 2 lane road. Will also occur when autopilot engaged as autopilot is simply TACC with lane keeping. The features otherwise seem safe on interstates and divided highways.

Summary

The defroster system is not able to keep the windshield clear of ice buildup when driving even in light snow. This is because this is an electric vehicle, so there is no engine heat from underneath the hood to radiate towards the wipers. Between wiper cycles, the wipers rest below the area of the windshield that is heated by the defroster. During snowfall ice builds up on the wipers from their resting position between cycles and that ice then propagates to the rest of the windshield as the wipers move. This is an extremely dangerous situation. I heard Tesla is installing a heater beneath the resting area of the wipers on the 2022 model year of this vehicle. However, NHTSA should issue a recall and require this heating plate be retrofit onto earlier model years. The country of Canada is aware of this issue as well and currently investigating it.

From June, 2019 to June, 2021 we owned a Tesla Model 3. We have had the Model Y since June, 2021. The current Model Y has a significant phantom braking problem. Worse than the Model 3 that we owned. Our Model Y relies on "Tesla Vision" without radar. Last week we drove 1,400 miles from Montana to Southern California. On more than a fifteen occasions phantom braking occurred, to the point that we were reluctant to use it. Driving on an Interstate at 80 mph, the car would suddenly brake. The 1st reaction is to overcome the braking with acceleration. On one occasion when there was no traffic behind, I didn't accelerate until 45 mph. From 80 mph to 45 mph suddenly. Dangerous. On city streets we also experienced phantom braking, yet not so dramatic. Some thoughts on what causes this issue: pavement glare, a change in pavement color, concrete barriers on an overpass, reflection from on coming traffic. Traffic Assisted Cruise Control is an important feature for me and now I'm reluctant to use it.

The vehicle unexpectedly brakes often while in cruise control when there is an oncoming vehicle in the oncoming traffic lane, despite the fact that both my vehicle and the oncoming vehicle are in their respective lanes (in other words there is no apparent danger of collision). The phantom braking also sometimes happens apparently at random, in other words when I can't see anything in the vehicle pathway that could be interpreted as dangerous. When using AutoPilot, the car will without any basis or immediate need brake most abruptly without any due cause, as there is no traffic or cars around it. It’s usually when passing over overpasses but not always. This sudden deceleration has almost caused accidents. Only me being wary and cognizant has saved the car from being rear ended. Sometimes there’s no cars around but it still can be very dangerous. This has been happening over almost a year and Tesla has not done anything to address this widespread issue.

On Jan 30, as an example, travelling on divided 4 lane highway, no one in front of back, autopilot engaged, for no reason, it tried to come to a sudden complete stop while going 70mph. There was nothing in the road whatsoever. MULTIPLE other instances of this. On 2 lane roads, this happens even more frequently, I would estimate once every mile, though not always as severe.

I was driving north on I-5 in southern Washington on a sunny Sunday morning with low traffic in October 2020. I had only had the car for a few days and was using the autopilot and cruise control for the first time. As I approached an overpass that was casting a shadow across the highway, the car suddenly braked hard. Fortunately there were no vehicles immediately behind me. If there had been, I likely would have been rear-ended. About 15 minutes later, while driving in the middle of the three northbound lanes, a motorcycle passed me on the left and then cut across my lane to get to an exit. Again, the car braked suddenly. When I next stopped to charge the vehicle, I went online and read on various Tesla user chatrooms that this was a widely experienced problem. I immediately stopped using the autopilot and cruise control. I have not used them at all in the past 15 months.

I have consistently experienced “phantom braking” when using cruise control. (I do not use full driver assist). I drive frequently on two lane highways and the braking occurs seemingly randomly, sometimes even when no other vehicles are close enough for concern, but usually when a large truck is approaching on the opposite side. I don’t recall having the problem while driving on divided highways. Each time this has occurred I am startled by the warning beep and am in fear of a rear-end collision, so I have stopped using cruise control except on multi-lane, divided highways. I read about “phantom braking” in the Washington Post and was surprised that there were not more similar complaints.

On numerous occasions while using cruise control the the Model Y brakes would be suddenly applied in the following situations. While in the inside lane of a curve in the road when several approached from the other direction the brakes would be suddenly applied when the front of the Model Y was pointing at one of the middle vehicles in the line of approaching vehicles. This sudden braking does not happing when traveling down straight sections of roadway or when traveling in the outside lane of a curve.

I just want to add my name to list to complain about phantom braking. You can't trust the adaptive cruise control to maintain correct speed as it will at random times brake hard when there is nothing in front of you. Maybe 20% of the times I use it.Sometimes it looks like a shadow on the road will cause it, maybe at night and when the weather is bad. It brakes hard too and you have to be ready to compensate. I think it is from using just cameras and no radar in my newer model

When driving on divided interstate highways using adaptive cruise control the car would suddenly without warning brake heavily. This is happening up to five times a day on long trips. Weather was clear, no other cars in front and no obstacles on the road. The only recourse was to slam the accelerator to the floor and hope no one was tailgating. This has also been happening on two lane roads when a truck or other large vehicle approaches. This was so dangerous we can no longer use the feature on two lane highways.

Driving a 4 lane road with not other vehicles in site the car suddenly braked to the point of locking up the seat belts and throwing items around the cabin. I reported this to the manufacturer and they were able to review logs and confirm that the car mistakenly saw something and applied the brakes. I would have been hot from behind if a car was following. I was told not to use this feature on certain roads. But I asked how to disable it I was told there is no way. This needs to be addressed before there is an accident. I was not using full self driving. I only had cruise control on.

It’s happened so many times at high speed, and while passing, that I am now scared to use “autopilot.” While using autopilot, I have to be ready to mash the accelerator peddle at any moment, due to unwanted breaking and it feels very dangerous. It can also be traumatic for passengers when the vehicle suddenly brakes hard at high speed. I’m worried it could also cause an accident if someone were right behind me and also traveling at a high rate of speed.

The contact owns 2021 Tesla Model Y. The contact stated that while driving approximately 55 MPH with the vehicle on auto pilot, a large commercial truck in the opposite lane passed the vehicle, which then applied the brakes and slowed from 55 to 35 MPH. The contact stated that this should not have happened and was an instance of "phantom braking". The contact reported that once the commercial truck passed the vehicle went back up to speed within seconds. The contact stated that he did not notice if a warning light was illuminated, but the contact stated that he heard the vehicle sound an audible beep. The contact did not take the vehicle to a local dealer to be diagnosed. The vehicle was not repaired. (1) My car dangerously and aggressively applies the brake dropping the speed of the car between 3 to 30 mph within a second or two (phantom breaking) EVERY SINGLE TIME I drive the car in adaptive cruise control or auto steer. This happens on 2 lane roads, on divided highway, driving on straight and curvy roads, when there are shadows, frequently when a truck or car is coming toward me, and even on overcast days when driving on a freeway when there are no shadows and no traffic in either direction. The car seems to be sensing an object that is going to cause a collision and applies the brake even when no object, threat, or shadow is anywhere in view in any direction. I frequently have to immediately step on the accelerator to keep from getting rear-ended in what could be a severe or fatal accident. (2) On a 225 mile drive to the Tesla Service Center to correct the phantom braking problem, my car inappropriately applied the brake 40 times, including aggressively dropping the car's speed from 63 mph to 35 mph in a second or two. (3) My car dangerously and aggressively applies the brake whenever a car is turning in front of me that is sufficiently far enough away that there is no absolutely no threat whatsoever of a collision. I then have to quickly step on the accelerator to keep from being rear-ended. (4) I have taken the car into the Tesla Service Center twice to correct this problem. I even gave them a list of dozens of times/dates I experienced phantom braking and the # of mph the car slowed (I recorded this on paper and as "bug reports" on the car's computer). On one visit the technician spent 12 hours reviewing my bug reports and the car’s operation. In both of my Service Center visits the bug reports were sent to a Tesla engineer for review. After both visits to the Service Center I was told the technician and the engineer could find nothing wrong with the car and “it was working as designed.” (5) In conclusion, my Model Y has a SEVERE safety defect.

What component or system failed or malfunctioned, and is it available for inspection upon request? - I assume the Tesla Vision System + Autopilot are what fail in this scenario. How was your safety or the safety of others put at risk? Immediately this puts us in risk of a rear end collision. Alert fatigue from false alarms creates a long term risk as drivers are trained to ignore / cancel alerts. Has the problem been reproduced or confirmed by a dealer or independent service center? Not explicitly. We have mentioned it to a dealer and said it was a common complaint. Also it is a well known (under addressed) issue amongst Tesla owners. Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? No, given the lack of property or personal injury it doesn't seem right to alert those institutions. Were there any warning lamps, messages or other symptoms of the problem prior to the failure, and when did they first appear? No, all systems appear to be functioning correctly. That is a large part of the problem - if the Tesla Autopilot is unable to function reliably on one lane highways it should at least warn drivers.

The full self-driving mode in my tesla does not react correctly to speed limit changes in small towns. I will be driving with the FSD at 65 MPH when I get to a small town and the speed limit changes to 45 and then down to 25 for residential area the cars speed does not decrease quickly enough to be at the correct speed limit by the time the car reachs the speed limit sign. Based on the speed limit laws I was taught was the car you are driving and are slowing down you must be at that limit when you reach the speed limit sign or else you can be ticketed for speeding. The Tesla FSD is not in compliance with that specific rule of the road.

The driver is becomes unaware of the true distance between the vehicle and objects behind it. This is worsened as the driver is unaware of the issue. I have experienced many episodes of sudden, unexpected and seemingly inappropriate strong braking. The most common circumstance is overtaking semi truck/ trailers that are slowly ascending the mountain slopes of Interstate 5 in southern Oregon. Often the trucks have their emergency flashers in use. IT self-brakes from 65 mph down to perhaps 30-35 mph and I "take over" by depressing the accelerator. It also happens frequently with oncoming traffic on 2 lane highways night or day and usually on a curve. The problem is frequent enough to avoid using cruise control on 2 lane roads and on I-5 mountain grades. It happens reliably on my way to town where an intersection has a flashing light and the approach is downhill into a right hand turn. It does not happen travelling the other direction and does not require any other traffic. For all of these, the sudden braking puts myself and others at significant risk for a rear-ender" collision. There are no lights, warnings, or ability to learn what triggered the inappropriate braking. I do not have the full self-driving option on this vehicle. A call to Tesla for service concludes with, " There are no faults with any of the hardware in the vehicle and these behaviors are a limitation of software in the system at this time". I did not take it to them to look at the car as it is 5 hours of driving plus the service time to do so.

When using Autopilot, I experience multiple, repeatable phantom braking issues on a regular basis. While it is worse on two lane highways, I've also experienced the issues on separated freeways and interstates. The phantom braking varies from a minor throttle response to decrease speed to full emergency braking that drastically reduces the speed at a rapid pace resulting in unsafe driving conditions for occupants of my vehicle as well as those who might be following behind me. Situations that seem to trigger the phantom braking include cresting a hill with oncoming traffic, navigating a turn with oncoming traffic, a larger than average vehicle (commercial truck/tractor trailer) approaching in the opposite late, shadows cast by large trees or overpasses, and changes in pavement color - for example, transitioning from gray concrete to black asphalt.

A well known phenomenon of "phantom breaking" occurs frequently and without warning with the Traffic aware cruise control engaged. This occurs not only on 2 lane highways, but even on undivided and divided highways. On the 17 mile commute to work everyday, this will occur 5-10 times, which results in rapid, unanticipated deceleration. At the time of these deceleration events, there is no visual cause to the driver that would have resulted. Often it appears to be activated by oncoming traffic lights, lights on the side of the road at businesses, when cresting the road, and when the road naturally has a bend or swerve in the traffic pattern. These rapid deceleration events place the occupants at risk of rear end collisions and also are uncomfortable as you must immediately place your foot on the acceleration pedal, which results in a jerking movement to the occupants. Telsa needs to either employ a "dumb" cruise control or greatly improved their TACC system before continued use of their system.

On a recent trip to Minnesota when adaptive cruise control was engaged there were several ( >10) episodes when the car would suddenly decelerate for no apparent reason. On a two lane highway it might occur if a large vehicle such as a truck was oncoming in the opposite lane. On the interstate, the common scenario was traveling in the right lane going up a slight upgrade with a car passing in the left lane. Often the sudden speed reduction was 25 mph or more. On occasion even on a flat section the car might suddenly brake for no apparent reason. On one occasion this happened in the afternoon going west with the sun glinting off some shiny asphalt crack filling. No accident occurred, but if a car were following closely, sudden braking could cause a crash. Because of this erratic behavior I don't use adaptive cruise control except for short distances. This is a known Tesla problem, but it seems the company is more interested in enhancing games to be played while charging rather than attend to a major flaw.

My Tesla Model Y seems to be suffering from what is called "phantom braking". My vehicle was produced shortly after the radar was removed and Tesla went to a camera only system. My car has the lasted updates installed. It is not a Full Self Driving vehicle. Since I purchased the vehicle it has had a problem during low light or sunny conditions with suddenly braking when using the cruise control. The situation has been reported to Tesla multiple times and "bug reports" were recorded and reported to the service center. During one morning drive of about 35 minutes I reported 8 occurrences. Sometimes the vehicle will sound the emergency avoidance braking. The car will suddenly slow from 60 to 30 mph. On multiple occasions the vehicle behind me had to take evasive action to avoid rear ending me. During one of my service visits the vehicle was taken on an extended over night test drive and the technician confirmed the problem. During the other visit the service center did not even take the vehicle for a drive. in both cases Tesla said the hardware was functioning normally and this was a "normal" operating condition for the vehicle. The car does better if I am following another vehicle because it seems to be able to maintain it's depth perception better if it has that reference point. I did report to the service center that the car seems to follow closer at night, even at the max following distance setting. The vehicle also struggles with the automatic high beam headlights which the computer turns on automatically if cruise control is activated. Very often the car is slow to turn the high beams off but in other cases it will turn the high beams off and then turn them back on as another car approaches. I am flashed by other drivers on a daily basis because of this. Tesla does not seem concerned with resolving these known issues. I hope NHTSA involvement can get their attention for these safety related items.

I have experienced several instances of the phantom braking in my Tesla model Y. it primarily happens when I’m on a two-lane road and there is a large truck approaching on the opposite lane. However it will also phantom brake on a highway where there are two lanes going in one direction separated by a median with two lanes going the opposite direction. It seems to happen when it’s coming up on a slight hill, where you cannot see over the hill. The car will slam on the brakes in the middle of the freeway, which when going at speeds of 65 to 75 miles an hour when traveling with other vehicles going the same speed, it can be especially dangerous in Texas where the giant pick up trucks like to tailgate electric vehicles. In one instance the pick up truck behind me thought I was brake checking him and jamming on my brakes to be a jerk; he then “coal rolled” me, putting out a giant cloud of black smoke. This situation has made it so I am afraid to use traffic aware cruise control, which I need to be able to use on road trips. It is especially bad in the two-lane winding roads around Amarillo, Texas, but I’ve also had it do this on a much larger road that goes through Chandler, New Mexico to Colorado. I absolutely love my Tesla, but the phantom braking is concerning, and the response I’ve gotten from Tesla service is that the car adapts and will eventually learn where not to do it. I don’t know if that’s true or not, but it doesn’t seem to be getting better, and it does it at the same locations on the road. I need cruise control to be working. Another suggestion might be to have the ability to turn off traffic aware cruise control. I paid for full self driving but have not had the courage to apply to use it because of phantom braking. I would also like to see federal laws passed to address coal rolling of electric vehicles. It is extremely dangerous and it’s happening a lot in Texas.

I’m experiencing “Phantom Braking” in my Tesla Model Y. I live in a rural area where 2 lane roads (1 lane each direction) are the only roads around. Larger pick up trucks, pickups with trailers, busses, semi trucks, dump trucks, sprinter vans, and other vehicles of that size in the opposite lane trigger my vehicle, when in cruise control, to slam on its brakes. This happens when it’s sunny, cloudy, any type of weather condition. Occasionally, when in cruise control, I notice the car makes slight braking attempts, even when there is no traffic in the other lane. It’s as if it’s sensing a ghost it doesn’t want to hit… This has happened ever since we purchased the vehicle. I do not use auto pilot because I do not see the point where I live. This issue happens when using the adaptive cruise control. I have not noticed it when using auto pilot as, mention before, I rarely use that feature. I recently say articles where many other Tesla owners were having this problem so I thought I would add to the awareness of this issue. My wife does not use cruise control nor auto pilot for fear the car will slam on it’s brakes and someone might rear end us. Speed limits range from 35-55mph in my area.

The vehicle driver assist system frequently has phantom braking in situations it should not be, especially when there are vehicles behind putting us at risk for a collision due to unexpected braking behavior of the car. This happens especially in setting of 2 lane roads with oncoming traffic, and guaranteed with a truck in the vicinity. On major highways it is less so, but does still happen occasionally. On local roads even if straight and no obstruction the automatic braking without notice occurs when least expecting. Also the lane departure system disables automatically for the remainder of a drive if you veer off a lane even once and didn't take the corrective action the vehicle thinks should have happened. That's not how a safety feature should work... It should remain on regardless. As much as I do enjoy this car, these quality issues definitely make you second guess and keep a closer eye on how you drive to avoid accidents if the system decides to not work as advertised

Yes. -Hard, unexpected, emergency braking at highway speeds from 75mph to immediate 35mph. Dangerous to passengers and other motorists due to the sudden, unexpected high deceleration speed. -Yes. Reported the problem to Tesla service center Spokane on 11/15/2021. They performed a remote investigation and told me I had used the car in a manner/location that was outside the current limitations of the software. No further action until the software system is updated sometime in the future and remotely closed the service appointment without any further input from me. - No. Phantom braking started the day I picked it up 9/24/2021 and continues to this day.

This car has had numerous phantom braking incidents. It is unusable on 2 lane roads. It breaks suddenly for trucks in the opposite lane. It can also begin to brake for cars in the opposing lane. The only remedy is to press on the accelerator to turn off the Autopilot. It also slows down periodically for shadows (for example when going thru an underpass). It was reported to Tesla service and evidently nothing can be done since it seems to be a software issue. I am unsure of that since this car no longer has the radar that previous versions of this car did have.

I was traveling in -5 degree (Fahrenheit) weather and the cabin heater stopped working (a notice appeared on the screen explaining that it was malfunctioning). Windshield began to fog up, so turned on fan and outside air in order to remove the fog. Ended up driving 8 hours in -5 degree weather with outside air coming in the car. Seat heaters and steering wheel heater continued to work. My passengers and I had to wear coats, boots, gloves, and a hat and still got numb hands and feet. I felt this was a dangerous situation, unable to keep warm while driving or when pulled over. Manufacturer looked at the issue and said the refrigerant was slightly low and replenished it. I don't feel confident that this was an actual fix.

The Tesla adaptive cruise control has been malfunctioning for the past 12,000+ miles (since new). Yes, it is available for inspection upon request. When the cruise is set, there are a significant number of phantom breaking events, making it a real hazard for anyone following our vehicle. The breaking can happen as often as 3 times every minute, and can decelerate 10-15 mph within seconds before I am able to react and override the cruise. The vehicle has been inspected 2 times by the manufacturer and has been found to have no hardware failure. There is no warning prior to the phantom breaking. However, periodically there are warnings during the phantom breaking events in the form of alarms sounding.

The adaptive cruise control system is very dangerous due to constant "phantom braking" problems. With cruise control enabled, the car will randomly slow down, sometimes gradually and others times rapidly. Sometimes the deceleration happens when passing a large truck, or a car on the other side of the road. Sometimes it is in response to shadows on the road. Sometimes there is no discernable reason. Instances of rapid and unexpected deceleration make it likely we will be hit by another car or otherwise cause an accident. This problem is prevalent throughout all Model Y cars and detailed online. Dealers say that there is nothing wrong with the vehicle and they hope it can be fixed in the future by software. However, in the meantime, the dangerous situation exists. My vehicle has not been inspected by anyone for this purpose, at least not yet. There are no warning lamps or messages when this happens.

The heat pump that supplies all heated air to the cabin stopped working completely. Safety was impacted because my windshield fogged up, making it impossible to see clearly out of my windshield or other windows. It was 11°F outside with no heating. I am not sure if a service center has been able to reproduce the issue, but I have images of the fogging and the error message. Nobody has inspected the issue yet. I received an error message "VCFRONT_a447" and "Cabin Climate Control System Requires Service" I have received this error message in two separate occasions now, both when it was lower than 15°F outside.

Summary

Hello, Tesla has released an update that will turn on the side camera when the blinker is activated. At night, the blinker can blind the camera making it difficult if not impossible to see any object. I'm concerned that this can cause the driver to turn into another car at night. The picture attached is with the hazard lights on and you can see how one camera works correctly and the other has a massive glare.

Sudden acceleration. I was stopped in my driveway (flat surface). The car started to roll forward and I pressed brake hard. Vehicle came to a complete stop but when I removed my foot from brake car lurched forward. Car hit a retaining wall approximately 3 feet away before I could stop it again by braking hard. When the car was put in park I was able to remove my foot from the brake without it lurching forward again. Little personal safety risk in this situation in my driveway with no other people around. the car hit the retaining wall at relatively low speed when it lurched forward.

I was driving on the 7 mile bridge in the Florida Keys with the Adaptive cruise control set to 58 mph. There was no traffic in my lane for about a mile ahead of me. As an oncoming Semi approached me, remaining in his lane, my car applied emergency braking. I had to immediately add throttle to avoid a collision with the truck behind me. This “ Phantom braking “ has occurred on multiple occasions to me. There is no way to engage cruise control without the adaptive feature which uses cameras instead of radar on these new Teslas. I did not have this problem on my 2018 Tesla that did use radar.

Received Error code: VCFRONT_447 indicating a cabin heat issue, and a complete loss of heat within the interior of the vehicle. It appears this also is preventing the battery from fully warming up causing severe vehicle power reduction and lack of regen braking. It being sub-zero and frost bite within minutes this poses a safety risk when traveling. The problem has been reported by other vehicle owners. It has not been inspected at this time. There were no other alerts prior to the issue starting on 1/20/2022 at ~6am CST.

While driving on ice and snow covered roads the regenerative braking feature comes in so strongly that it makes driving dangerous as the car loses traction and control every time the foot is removed from the accelerator. The regenerative braking automatically applies once the foot is removed from the accelerator. Tesla used to have an option to reduce the strength of the regenerative braking but removed that option in October of 2021. That change in software has made the vehicle unsafe to drive on slick roads. I was able to maintain control of the vehicle because of my driving skill, but did lose control numerous times during a 50 mile trip because of the increased regenerative braking feature. Tesla vehicles manufactured before October 2021 still have the option to reduce regenerative braking power, I also own one of those vehicles. I have attached a photo of that screen that shows the option.

The cruise control and auto pilot functions failed while driving in the rain. The car would not disengage from auto pilot and continually would engage back into auto pilot and eventually locking tires and spinning out across all lanes of the freeway. My safety and all the drivers around me are put at risk when something that dramatic occurs. The car spun out completely out of control and after doing a complete spin the car continued to spin losing all control and ended up inches from the center median and was facing oncoming traffic. Unknown- I instantly called tesla and removed my memory stick from the vehicle, they are still conducting their "investigation". Took it into the dealer and had them look into the issue. They are currently remotely working on what caused the issue with their engineers. No warning lamps or messages appeared prior to the occurence.

Several times now the car has done a phantom braking event where the car brakes for no reason when using adaptive cruise control on two lane highways. Mostly it's when large vehicles like a semi truck or RV passes in the other lane. Sometimes it appears to be due to shadows like overpasses or trees. The car slows down so quickly sometimes that if someone were following me it may cause a rear end collision. Most of the time the slowdown happened without warning. Last year Tesla switched to a heat pump system for heating the interior of the car. Since that change the 2021 models are regularly loosing heat when the temperature gets below zero. Sometimes restarting the car will bring it back but not always. This can lead to drivers being stranded with no heat in potential dangerous temperatures. The heat stops working when the ambient temperature reaches 0 degrees. This is a safety risk in cold temperatures, especially for young children that can’t wear coats in the car due to car seats. This has happened in newer models that have a heat pump as the heating system, per social media accounts. The service center is inspecting tomorrow. A warning message appears when the heat stops working on the car screen. There is also an error on the phone app when preconditioning cannot start (preconditioning is supposed to mitigate the error, but it gets too cold to precondition).

I was driving on interstate 285 Atlanta in rain. All of a sudden the screen froze. Wipers won’t work. Had poor visibility. Could not even put on emergency lights. Had to do hard reset and pull over. Before I could get them to work again. This was extremely dangerous in the middle of the road. Similar issue happened with my wife in a brand new model 3 a few weeks ago and we were told it was a software issue and resolved with an over the air update. I have the UpToDate software in my model y. Feel Tesla softwares should be approved by NHTSA before being sent to customers.

The backup warning system did not work when I backed out of a driveway on a dark night. I knew there was a black car (sedan I believe) parked along the curb across from the driveway, and could partly see it in the backup camera when the tail lights would turn on (i.e., when I stepped off the drive pedal), but was unable to tell exactly how close I was to the car. There were no beeps from my car to warn me. My car stopped abruptly as the edge of my trunk lid hit what I guess was the corner of the other car. All damage appears to have been to my car - a dent in the trunk lid. I reported all of this to Tesla through their service app, and received a list of places from which I can get an estimate to repair the damage, but never received an explanation from them of their evaluation of the car's records I want to point out that I believe my Tesla includes all camera sensors - a change they apparently made over the summer. My car was ordered in June 2021 for September delivery.

Tesla recently overhauled the firmware to their vehicles, however the newest update removes much of the ease and functionality. They have removed defogger buttons into the background, and other settings as well, causing the user to have to use multiple touches to work something that previously was quick. I believe this to be a safety issue. Drivers are now unaware where certain settings are and causes drivers to become distracted from the road to a degree that is unsafe while driving.

With Tesla’s new software version 11.0 (Firmware 2021.44.25.2), the enabling of the front and rear window defogger is not readily accessible (they are buried in a menu). In the previous version of the software, there was a dedicated button on the screen to enable each of them. This is a safety issue because of the following scenario that happened to me today, comparing to my experience just the day before: I was driving in the mountains of Colorado, where it was below freezing and snowing. On the 22nd of December, BEFORE the software update, my front window started fogging up VERY quickly. I was able to turn on the front and back window defoggers with one tap. Today, I was driving back and the same thing happened — the window started fogging up. With the new software you had to tap twice, taking away your attention from the road in order to locate the defrost ON button. This is incredibly dangerous. I opted to move to the side and put my flashers on while I did that.

When driving on a two lane road and approaching an oncoming 18 wheeler the cruise control consistently slams on the brakes and I am concerned about the well-being of my 5 and 4 year old in the back seat. Traffic Aware Cruise Control is just about useless on 2 lane highways. Once enabled it is plagued by frequent decelerations for absolutely no obvious reason. What is more concerning is the application of emergency braking when meeting an oncoming truck, even on a well marked highway where the truck is centered in it’s lane. Fortunately this has not happened to me while I was being followed by a vehicle, but it is highly likely that a large truck or inattentive driver would have rear ended me in this scenario. Tesla service has looked at my car, and told me that everything was working properly, that it was a common complaint and they did not have a fix.

A right banked curve on a two lane road in a rural area came up with a vehicle coming around the curve the opposite direction. The Model Y slowed slightly and began making the turn properly and without cause for concern. Suddenly, about 40% of the way through the turn, the Model Y straightened the wheel and crossed over the center line into the direct path of the oncoming vehicle. I reacted as quickly as I could, trying to pull the vehicle back into the lane. I really did not have a lot of time to react, so chose to override FSD by turning the steering wheel since my hands were already on the wheel and I felt this would be the fastest way to avoid a front overlap collision with the oncoming vehicle. When I attempted to pull the vehicle back into my lane, I lost control and skidded off into a ditch and through the woods, causing significant damage to the vehicle. I was able to retrieve the dashcam footage of this incident and it is attached here. I did analyze the footage myself briefly. You can see the vehicle leave the lane, and within about 10 frames, I had already begun pulling back into the lane before losing control and skidding off the road. Since this video is recorded at about 36 FPS, this would mean I reacted within 360ms of the lane departure. I understand it is my responsibility to pay attention and maintain control of the vehicle, which I agreed to when I enrolled in FSD beta. I was paying attention, but human reaction does not get much faster than this and I am not sure how I could have otherwise avoided this incident. I am thankful this was the outcome rather than a front overlap collision with the oncoming vehicle, which would have likely resulted in bodily injury. Tesla has confirmed that FSD beta was engaged, per the attached correspondence, but insists that this incident and the resulting accident are my responsibility.

When cruise control or Autopilot systems are active, the car will often slam on the brakes and sound an alarm as if there going to be an imminent collision, even thought the lane is clear in front of the car. This happens most likely often when cresting small hills or on curves. There may be a car in coming in the opposing lane, but there is no danger as the opposing car is fully in their lane. The adapative cruise control or Autopilot system decreases the speed very quickly, often reducing the speed by 10-20mph within seconds. This causes panic for the driver and any cars behind us have to suddenly slam on their brakes, creating a dangerous situation. I own both a 2021 Tesla Model Y and a 2021 Model 3 that both have the Tesla Vision Camera based cruise/Autopilot/collision detection system. This problem has gotten worse as Tesla has released new firmware updates in recent months. Many Tesla owners are reporting that they are having the same issues recently. Tesla's response is that the systems are under development and that they will improve over time. The problem is that this is a safety issue NOW! They seem to be overly focused on developing and testing their full self driving systems that they are ignoring a huge safety flaw in their current cruise control and Autopilot systems.

My wife and I purchased our Tesla model Y back in February 2021. We picked it up back in May 2021. The selling point for us was how it drove in the winter since we live in central WI. When we test drove the car in a snow snow at the highland park dealer. The car handled great in the ice and snow due to the car having the winter driving option. My wife nearly got into an accident due to no longer having the option for winter driving. The regenerative braking option is great until you drive in winter conditions. Ice and snow will cause the vehicle to slip and slide whenever letting off the throttle. The safety concern is that if a line of vehicles is in front of you and one person slams on the brakes letting off the throttle is essentially the same thing as slamming on your brakes. Which you never do while in a snow storm. We essentially bought the vehicle with and for this option to to control the vehicle better based on weather conditions. Removing the the winter driving option was not only wrong but a law suit waiting to happen due to the safety issue it brings.

On two occasion, our brand new Tesla model Y lurched forward from a complete stop. The first time, on 2021/10/09 at 4:36pm I was at a car wash and trying to put the car in neutral. Fiddling with the gear selector, the car jumped forward without me touching the pedals. I hit the breaks and neither the car or anyone else was injured. The second time, on 2021/10/25 at 8:10 am, my wife was dropping off the kids at school. At full stop the car lurched forward again without her touching the pedals or the speed controller. The car hit the car in front of it 3 times with force. The car as well as the two other cars in front of it got seriously damaged. My two year old son was on board, there was many middle schoolers around but thankfully no one was injured. Incidents were reported to Tesla right away who has assured us they would investigate but we have had no update since despite contacting them multiple times. They assured us they have the ability to download telemetry remotely but I am concerned that as time goes by data will no longer be available. We later got an alert on the app that the emergency braking system had been disabled at the time of the second incident. I have videos files, recorded by the car itself of both incidents as they are happening available. The body damage of the car is fully repaired but the car is otherwise available for inspection/data downloads.

While driving on an interstate highway (75 mph) at about 12am (little traffic) on four separate occasions, with cruise control engaged, the brakes were unexpectedly applied without warning, no forward traffic, no wildlife, no vehicles on the side of the road. Had there been a vehicle behind us, it could have resulted in a rear end collision. Tesla's autopilot / adaptive cruise control / emergency braking is defective. On multiple occurrences I have had incidents where the vehicle's adaptive cruise control and automatic emergency braking systems have activated inappropriately. In one occurrence I was driving on I-17 through Phoenix in the middle lane. I was beginning to pass a pickup truck pulling a trailer (my front bumper was just behind the trailer) when the car detected this as an imminent collision and applied significant braking. I had to use the accelerator to disable the emergency braking, and this triggered a number of cars behind me having to brake as well. In a second occurrence I was driving on SR-74 westbound in phoenix between I-17 and Lake Pleasant Parkway. This is a two lane road in many sections, and at one point I had a semi truck and trailer coming towards me in the opposite lane and my car detected this as infront of me and applied emergency braking. Thankfully there was no one behind me as the vehicle reduced speed from 65 to 40 before I was able to push the accelerator and resolve the issue. There have been other incidents of it braking in ways that were unexpected and dangerous. What component or system failed or malfunctioned, and is it available for inspection upon request? Adaptive Cruise Control breaks hard for no apparent reason. If there had been any cars trailing close behind I could easily be rear-ended. Happens at least once every time I turn on cruise control. On a recent longer highway trip of about 90 miles it happened three times. One time on that trip it happened while passing under a bridge, did it pick up the lower speed from the bridge above? The other times it seemed to happen for no reason. All these times our hearts dropped (2 passengers in car). Note that I do not have full self driving, so I’m talking about adaptive cruise control being the problem.

When driving with Cruise Control engaged, the car repeatedly brakes VERY HARD. Sometimes the braking is when there is a car in the opposing lane and is approaching but not over the line. Sometimes the sudden hard braking occurs for no apparent reason. But on a two lane highway this occurs VERY regularly. It occurs several times an hour. This is a serious problem because if there is a car behind me the sudden hard braking may cause that car to crash into me from behind. You can submit a bug report to Tesla by pushing the right hand steering wheel button, saying "Bug Report" and then describing the report verbally while driving. This will include all sorts of driving data being sent to Tesla. I have done this dozens and dozens of times. Each time I get a message on the screen thanking me for sending in the bug report. Never once have I ever heard back from Tesla other than the instantaneous message on the screen saying that the bug report has been submitted. I believe that Tesla has had a recall for a very similar problem with the Model S and Model X with Full Self Driving. This recall was fixed almost immediately with an over-the-air software update. However my problem does not relate to Full Self Driving (which my car does not have). This relates only to cruise control. I have spoken to several other Model 3 and Model Y owners who ALL report the exact same problem. A quick search online will reveal hundreds of other similar reports for Model 3 and Model Y owners who do NOT have Full Self Driving. When on Cruise Control or using Autopilot mode, there are frequent phantom breaking issues. On a long trip, we experienced phantom breaking consistently on the Highway both at night and during the day. These phantom breaking issues would sometime be significant and immediately drop speeds 10 miles an hour creating a danger of surprising people behind us. On a 2 lane country road, the issues are GREATLY compounded. The breaking would constantly trigger with oncoming traffic in the other lane and break VERY hard with the auto break alarm sounding. At night on a country road, it occurs with almost EVERY oncoming car. It is amazing to me that this made it through quality control because it is easily reproducible. The auto braking feature should just be turned off for country roads if this can't be fixed. At one point, I was right next to an older Model 3 on a mountain passing lane when an oncoming car was coming. The Model 3 was in the left lane closer to the 'danger' and my car phantom braked with the auto breaking alarm coming on and the model 3 just continued on, no issue. No idea if they were on cruise or auto pilot etc.

On September 26, on our first long distance trip with our just delivered (9/17/2021) Tesla Model Y, during daylight nice weather driving with Adaptive Cruise Control switched “On”, on at least 6 occasions (strait roads, freeway and non-freeway driving), the car suddenly, without any reason, violently slammed on the brakes. These events are hair raising for me and passengers, let alone for a driver behind me. If he/she does not pay attention at that very moment, the result could even be disastrous. I would never have expected such a serious safety issue with a Tesla. Suffice to say that this is absolutely unacceptable to me. When checking out the web, many people reported similar issues with their Model Y vehicle. I had the car checked-out at the Tesla Lynnwood, WA, Service Center on September 30, 2021, and when picking-up the car I was informed that no anomalies had been found. At later dates, similar emergency braking occurred without any reason, while driving with Adaptive Cruise Control. Obviously, I do not trust this Cruise Control system one bit anymore, and no longer use it. I have written a letter to Tesla (11/12/2021) requesting a time frame when a solution can be expected, but so far, I have not received any word from them. I sincerely feel that Tesla should temporarily disable this Adaptive Cruise Control feature on ALL Model Y vehicles currently on the road and those still to be delivered (to unaware new owners), until this serious safety issue has been resolved.

Driving across the country in Model Y. Passed semi in left lane, and when merging back into right hand lane vehicle applied full brakes dropping the vehicle speed from 80mph to 60mph, right in front of semi. I was forced to take over and accelerate to avoid serious rear end collision. It wasn’t just the one time, vehicle continued to perform aggressive braking actions many times. Autopilot was unusable for majority of trip. Tesla needs to make autopilot able to be disabled, and standard cruise control possible. Tesla vision is not safe, and is a major liability. It’s very clear Tesla vision is an attempt to remove radar to get around supply chain issues, at the cost of consumer safety.

My car gave a red color warning of imminent collision and braked hard. Fortunately, I avoided having an accident with cars following me by pressing on the gas peddle and moving ahead. Tesla service remotely reviewed and determined that at that time emergency brakes were applied because the car detected an object. Tesla said it could have been a reflection, the sun, or any number of things that could have triggered the false detection causing the vehicle to brake.

Very often when "autopilot" or "traffic aware cruise control" are activated on two lane roads (county or state highways) and there is oncoming traffic the system will alarm and brake hard as though the car is about to have a head on collision. This is a safety hazard because if anyone is following me when this happens it would be very easy to get rear-ended. This problem has been happening since I purchased the car and has been reported to Tesla 4 times along with videos of the incidents and they refuse to fix it.

Sudden acceleration while stopped. Car was stopped in the driveway of my home while waiting for the garage door to open. Suddenly the car bolted forward, crashed through the rising garage door and then through the side of the garage and into the front steps of my home. The car is equiped with auto pilot, but it was not engaged. While reviewing other incidents of this type on your website, I noticed that other people mentioned their Tesla's having the autopilot feature but I saw no mention of the Model Y. Although extensive damage happened to the car and my home, no one was injured. However if this had happened while at a stop light or at a crosswalk someone could have been killed or injured. Tesla delivered a car with a concerning mechanical defect on all four hinges of both rear/forward doors. There are deep gouges to the metal on all four hinges. They have no explanation as to why the metal that holds the doors I place has been damaged. Multiple calls to Tesla has resulted in them saying they will absolutely not look at the issues on the car until December 10th. They said that I can't even bring it in to a service center for safety inspection. I wanted to reject delivery and get a safer car but they said that I would then not receive a car for 4-6 months later. Meanwhile they had already accepted payment, so I would be on the hook for payments. I don't know if these.doors are safe, I'm looking for a Tesla to say whether the car is safe to drive or not, and find out what caused the damage. Tesla will only tell me that they will look at the car in December.

I had 2 accidents both caused by the wheel forcefully turning and not letting me take over. First incident: On 8/31/21 I was driving on the right lane of the palisades pkwy S (NJ) the car was on “autopilot” my hands where on the steering wheel. It was raining, there was no car around me for at least a few hundred feet. Suddenly the “forward collision warning” started beeping, and I felt the steering wheel is forcing itself very sharply to the left, I tried turning back to the right, but I couldn’t. Fearing that the car will end up in the lane of the opposite traffic, I slammed the breaks. The car ended up doing a 180 hitting the dividing guardrail against oncoming traffic. The car was towed to a shop where it was until oct 18 when it was ready for pickup, damage costed about 15k to repair. There was no injuries, and no other car involved. Second incident: On oct 26 A week after I picked up the car from the repair shop, I was driving on the NJ Turnpike N on the left most lane. It was raining, and the car drove over a small puddle of water. Suddenly I felt the steering wheel turning very forcefully to the right. I tried pulling back to the left, but was unable. it felt as if the steering wheel was locked, causing the car to abruptly make a hairpin curve, bypassing the next lane, and hit a car two lanes over. From the impact the car was thrown back to the left, knocked into the highway divider, the car then spun and flew all the way to the other side of the highway. This time there was another car involved. I was in the car with my wife, both of us besides being all shaken up had no injuries, it is not known to me if there was any injuries to the other driver My car was a total loss. In both instances I felt the car taking over the steering wheel and not letting me take back control. The first incident was on autopilot, second time was not. Our 2021 Model Y features the new vision based cruise control system, both traffic aware cruise control and autopilot utilize this camera only system. At the time of ordering we were under the impression we were getting a much safer and mature radar based system. Upon accepting delivery at the end of May we have accumulated 9,000 miles on the car and have a had horrible experiences with the traffic aware cruise control slamming on the brakes for no apparent reason with nothing ahead or passing cars. Behavior can be 5-10mph slowdowns or in some cases FULL brake pressure which puts us in danger of being rear ended. Multiple times we have been close to rear ended. In addition the system can also produce audible beeps when cars pass however no warning or message is displayed. I want to stress this behavior is a daily occurrence day or night. We have reached out to Tesla in order to get the vehicle looked at for repairs and have a been denied even a service appointment. The vehicle was declared fine hardware wise. I take great exception to the practice Tesla is utilizing of declaring previously standard features "BETA" and making customers deal with their poor results with no accountability. We don't accept BETA seatbelts or BETA tires. This feature was enabled by default on the car upon purchase and it objectively doesn't work safely. This compliant is really an act of desperation as we simply have no other avenue to express our frustration and safety concerns for a half baked feature.

In Feb. 2021 I purchased a new 2021 Tesla Model Y. In August I noticed a crack in the glass ceiling originating from the front edge of glass. We made an appt for 10/19 to have it replaced at the S. Seattle Tesla Service Ctr, assuming the repair would be covered under warranty. We brought the car in on Oct 19. The service rep inspected the crack and very quickly determined that the crack was a result of something hitting the ceiling such as a small rock. The rep's eagerness with her response suggested that she was familiar with this issue and was ready to deny warranty coverage. She told us we should make a claim against our auto insurance policy. We countered that we didn't believe anything had hit the ceiling, and even if it had, the glass should not have cracked that easily. Also, we also pointed out that in all the other cars we've owned we have never experienced rocks being spun up from the road hitting the ceiling. The rep still declined. We went ahead with the repair. We searched the internet for other Tesla owners whose ceiling glass had also cracked and found a number of postings on several forums of customers who had the same disappointing experience we had. It is evident that there is some kind of design flaw that results in the ceiling glass cracking easily. We inquired with a class action law firm who informed us that there is a provision in Tesla's standard warranty in which the consumer agrees not to enter into any class action lawsuits against Tesla and requires arbitration to settle disputes.We wanted to bring this issue to your attention attention because we believe Tesla is well aware of a design flaw that results in the cracking of their glass ceilings and is concealing their knowledge of the frequency of this problem.

At random times, without warning, cruise control changes from set speed (e.g. 70mph) to 45mph and vehicle immediately begins hard braking to achieve new set speed. Any cars following may not be able to react quickly enough to avoid a collision if the driver doesn't react quickly enough to command acceleration back to freeway speed. I sent multiple videos of this happening to Tesla's service department. Tesla virtual service has looked at vehicle's logs and says there's nothing wrong with the vehicle and that it must be in how the vehicle is interpreting fleet data and map data. Clearly a software bug not validating it's input for reasonableness. No audible warning and while the cruise set speed does change on the display, the deceleration happens simultaneously, so no actionable visual warning is given to prevent the uncommanded deceleration and one must react immediately.

However I was told that it wasn't a hardware issue but a software issue. They will issue a fix however it has been 4 months with no fix insight. While leaving a parking lot, the vehicle lurched and felt like anti lock brakes kicked in and the car stopped working. The contact stated that while driving approximately 70 mph but doesn't recall; the vehicle began shifting from left to right for 5 - 10 seconds when another vehicle approached the passenger side of the contact's vehicle. The contact attempted to disable the autopilot by depressing on the brake pedal and maneuvering the steering wheel however, the vehicle would not allow the contact to control it and it spun around and hit the median wall on the highway. Medical attention was received on the scene for the contact's sister, who was a passenger. A police report was filed. Backing out of garage, the driver began to smell acid and burnt electronics from the back of car. After driving back home from work the driver inspected the car. The back driver side door reeked of burnt electronics and the entire car had an acid smell. After consulting Tesla, the first Tesla maintenance worker noticed the entire inside of the door had melted and all electronics were fried. He claimed he had never seen this before. The second maintenance worker came with parts and determined the window module had overheated and caught fire. The fire melted many components inside the door. A brand new 2021 Tesla Model Y should not spontaneously catch fire. Especially if it is residing inside the garage of a house. After replacing all the components, Tesla deemed the car safe to drive. Yet, a couple days later they requested we bring the car in so that they could replace the computer that controls the left side of the car to do more research and find the 'root' cause of the fire. There were no warnings for the driver, nor was the driver using that window module. We have not driven the car since as we feel unsafe in the vehicle.

The display screen (eMMC) crashed. The contact attempted to reboot the system however, the touch screen failed to respond to the touch. The vehicle was taken to the dealer, but the dealer could not duplicate the failure and informed the contact that everything was fine with the vehicle. The failure recurred. The vehicle was taken to another dealer Tesla (3067 N Elston Ave, Chicago, IL 60618) and after the dealer test drove the vehicle, the mechanic found out that there was a firmware bug that caused the failure. I pulled up next to a parking space in a parking lot and the Tesla auto park feature was offered on the screen after detecting the parking spot. I pressed to auto park and the tesla proceeded to perform the auto park. During the parking the tesla hit the truck parked in front of the tesla while turning into the space and broke the taillight of the truck and scratched the tesla on right front side. I then braked and disable the auto park after the collision. I reported the incident to Tesla and they reported back that they don't see any issues with the sensors at that time and they say I need to take full responsibility. I cannot think of anything that was particularly challenging for this parking spot or surroundings. It seems a bad calculation by the sensors of Tesla or the software and it made a mistake. Given this experience I cannot trust any of Tesla autopilot features now. They have concluded they will not do anything about it.

This problem happens repeatedly. It has been reported by multiple Tesla owners but no action has been taken by Tesla. When driving with the cruise control engaged, the car will brake very aggressively. This happens most commonly when there is no other traffic on the road and no debris on the road. It seems the car believes there is something in front of the car for which it needs to stop, even though the road is clear. But if I press on the accelerator the car resumes normal cruise operation. If there is a car behind me I am worried that the car behind me will crash into me. The braking action is VERY aggressive.

Wiper failure when set to automatic (rain-sensing) when vision system shut down due to poor visibility. A little background: I've been driving for 50+ years, about 1.5M total miles, most of it commuting. I'm a software engineer with 45+ years of experience, mostly in embedded systems. Problem drive: most settings, including headlights and wipers, on auto. Autopilot was on. Drove into a heavy rainstorm, auto wipers worked well, speeding up until they were on high. Traffic speed dropped from 70+ to less than 35 due to rain. Drove into a band of torrential rain. Saw a pop-up on the screen that said something like autopilot turning off due to diminished visibility. Autopilot (speed control) then turned off. THEN THE WIPERS SHUT OFF. Let's see, guy designing vision system: can't figure out anything from camera images, so let's warn user and shut off stuff that relies on the camera. Like autopilot. Wipers? Leave them in the last state? Nah, turn off the enable signal to the auto wiper state, too. Torrential downpour, almost no visibility, and my wipers just shut off. I frantically kept pressing the wiper button on the turn signal stalk to force a couple of swipes and force the wiper pop-up menu, then take my eyes off the road with possibly someone stopping right in front of me (yes, I slowed down) to do hand-eye coordination on the wiper pop-up to hit the manual setting full speed button! Related to Tesla Model S/X recall for touchscreen failure disabling defog/defrost. Proper design requires hard controls (not just touchscreen) for safety-critical items, like lights, wipers, defog/defrost. That's why a hard control for hazard flashers is required. I've spent years designing life-critical medical systems. They all have hazard mitigation matrix sections - analyze possible fault modes, assign chance of happening and severity of impact if they happen, then have a mitigation strategy. This seems to be a system design flaw

The same week I purchased my Tesla Model Y, the screen would freeze and the touchscreen would cease to accept input. I took it into the service center twice in the same week and the cause turned out to be a pinched cable harness which was repaired under warranty. However, I would like to submit my feedback that I was unable to operate basic functions of the vehicle including windshield wiper control, mirror adjustments. However, the car remained driveable by shifting the drive stalk. I have also observed the screen would sometimes freeze which needs a reboot. If I’m driving in a heavy rain and this happened , my family’s safety could be compromised by not accessing basic functions required for safe operation of the vehicle. Tesla promptly repaired the issue but if it would happen again, I would not be able to use basic car screen functions where a lot of safety controls reside.

THE MIRROR ADJUSTMENTS DO NOT ALLOW VISUALIZATION OF THE LANE ON THE LEFT OR TO THE REAR WITHOUT BENDING TO THE SIDE AND CRANING MY NECK. IT CREATES AN UNSAFE SITUATION WHILE DRIVING AND TESLA SAYS THERE IS NOTHING WRONG. I HAVE TRIED EVERY POSSIBLE ADJUSTMENT AND IT IS A CLEAR DESIGN FLAW. WHEN THE GAS PEDAL IS RELEASED AT SPEEDS GREATER THAN 15MPH, THE REGENERATIVE BREAKING SYSTEM IS ENGAGED TO SUCH A DEGREE THAT THE TIRES BREAK TRACTION WHEN DRIVING ON SLIPPERY SURFACE LIKE SNOW OR ICE. THE CAR THEN GOES OUT OF CONTROL AND SLIDES IN AN UNCONTROLLED MANNER. THE CAR HAS VARIOUS SETTING FOR BREAKING, BUT EVEN THE SETTING THAT OFFERS THE LEAST BREAKING (CALLED "ROLL"), RESULTS IN SIGNIFICANT BRAKING ACTION WHEN THE GAS PEDDLE IS RELEASED AT SPEEDS ABOVE 15MPH' ENOUGH FORCE THAT DRIVER AND PASSENGERS FALL FORWARD AGAINST SEAT BELTS. MY EXPERIENCE WITH MY 2-WEEK OLD CAR IS THAT THE CAR WENT OUT OF CONTROL, AND I SLID ACROSS THE ROAD INTO AN EMBANKMENT. I HAVE 30 YEARS OF INTENSE, REMOTE WINTER DRIVING EXPERIENCE, AND I HAVE HAD ZERO ACCIDENTS, UNTIL NOW IN A BRAND NEW TESLA. THE CAR SHOULD OFFER A SETTING WHERE NO MECHANICAL OR REGENERATIVE BREAKING IS APPLIED WHEN THE GAS PEDAL IS RELEASED. ALTERNATELY, THE ANTI-LOCK SYSTEM SHOULD BE ACTIVATED WHEN THE GAS PEDAL IS RELEASED SO THAT THE BREAKING SYSTEM CAN DISENGAGE IF WHEELS STOP ROTATING (IF A SLIDE/SKID IS DETECTED). IT'S A HUGE FLAW THAT NEEDS TO BE ADDRESSED IN THIS OTHERWISE IMPRESSIVE CAR. HERE'S THE GIST: ALL GOOD DRIVERS KNOW THAT IF YOU PRESS A CAR'S BRAKE WHILE ON SNOW OR ICE, (PARTICULARLY IN A TURN OR WHEN THERE ARE OTHER INERTIAL FORCES) THE CAR WILL GO OUT OF CONTROL. HOWEVER, THE TESLA MODEL Y GOES OUT OF CONTROL WHEN YOU LIFT YOUR FOOT OFF THE GAS ' NO BRAKE NEEDED. THIS IS A NOVEL BEHAVIOR AND THE CAR SHOULD EITHER NOT DO THIS, OR THERE SHOULD BE A SIGNIFICANT WARNING TO NEW DRIVERS.

THE CLUNK CAN BE FELT AND HEARD WHILE TURNING LEFT ONTO DRIVE WAY. SERVICE WAS ABLE TO REPRODUCE NOICE BUT WAS NOT ABLE TO PINPOINT THE CAUSE. TECH DID STATE THAT A BOLT WAS 1/8 OF A TURN LOOSE. TECH PERFORMED ALIGNMENT AND STATE THST NOISE WENT AWAY. SERVICE ADVISOR RELEASED VEHICLE AND ASK TO INFORM HIM IF PROBLEM RESURFACE. SAME CLUNK NOISE PERSIST BUT AT A LOWER NOISE LEVEL WHILE TURNING LEFT ON DRIVEWAY.

HOWEVER THIS LEADS TO A WORST SITUATION WHERE WE SEE ONLY THE GROUND NEXT TO THE VEHICLE INSTEAD OF CARS OR OBJECTS ON THE REAR LEFT. THIS IS A RECENT ISSUE INTRODUCED IN MODEL YS WITH AUTO-DIMMING MIRRORS ONLY AND I BELIEVE THIS WILL CAUSE ACCIDENTS IF NOT ALREADY IF TESLA DOES NOT RELEASE A FIX FOR THIS ISSUE.
